血液透析患者C反应蛋白水平与心脑血管疾病的关系 被引量:1

The relationship between CRP in hemodialysis patients and the development of cardiovascular disease

摘要 目的:观察维持性血液透析(maintenance hemodialysis,MHD)患者C反应蛋白(C reactive protein,CRP)水平与心脑血管疾病的关系。方法:选取2014年7月—2017年7月在南通市第四人民医院血液净化中心进行MHD的尿毒症患者40例,按CRP水平判定是否存在微炎症状态分为微炎症组(n=20,CRP>6 mg/L)和非炎症组(n=20,CRP≤6 mg/L)。另选取我院同期健康体检合格者20例作为对照组。结果:MHD患者CRP水平显著高于对照组(P<0.05);微炎症组患者心脑血管疾病的发生率和总死亡率显著高于非炎症组(P<0.05);微炎症组患者血红蛋白(hemoglobin,Hb)、血浆白蛋白(albumin,ALB)、高密度脂蛋白胆固醇(high density lipoprotein cholestero,HDL-C)水平显著低于非炎症组,而低密度脂蛋白胆固醇(low density lipoprotein cholestero,LDL-C)水平显著高于非炎症组(均P<0.05);MHD患者LDL-C水平与CRP呈显著正相关(r=0.535,P<0.05),而Hb、ALB、HDL-C水平与CRP呈显著负相关(r=-0.418、-0.587、-0.552,均P<0.05)。结论:MHD患者的微炎症状态与心脑血管疾病的发生密切相关。 Objective:To investigate the relationship between C reactive protein(CRP)in hemodialysis patients and the development of cardiovascular disease.Methods:40 hemodialysis patients with uremia in Fourth People’s Hospital of Nantong were collected from July 2014 to July 2017 and divided into microinflammation group(n=20,CRP>6 mg/L)and non-microinflammation group(n=20,CRP≤6 mg/L).20 healthy people from the same time were termed as control group.Results:the CRP level in hemodialysis patients was higher than that in control group(P<0.05).The morbility and mortality of cardiovascular disease in microinflammation group were higher than that in the non-microinflammation group(P<0.05).The hemoglobin(Hb)and albumin(ALB)high density lipoprotein cholestero(HDL-C)in microinflammation group were higher compared with the non-microinflammation group and low density lipoprotein cholesterol(LDL-C)level were lower than that in non-microinflammation group(P<0.05).The LDL-C level in hemodialysis patients were significantly positive related with CRP level(r=0.535,P<0.05)and Hb,ALB,HDL-C in hemodialysis patients were significantly negative related with CRP(r=-0.418,-0.587,-0.552,all P<0.05).Conclusions:Microinflammation in hemodialysis patients were closely related with the development of cardiovacluar disease.
