摘要
目的:明确稳心颗粒联合美托洛尔治疗非瓣膜性房颤的临床有效性和安全性。方法:以主题词结合自由词、计算机检索结合手工检索的检索方法检索中国知网(CNKI)、万方数据资源系统(WANFANG DATA)、中文科技期刊数据库(VIP)、Pub Med、Embase、Cochrane及其他中医药相关期刊中相关的文献,时间截至2017年5月,收集稳心颗粒联合美托洛尔治疗非瓣膜性房颤疗效的随机对照试验(RCT)文献,严格按照纳入标准筛选文献,并根据Jadad量表的质量标准和Cochrane协作网偏倚风险评估工具对纳入文献的质量进行评价;提取文献中的原始数据,客观评价其疗效。共纳入24项研究的1938例患者。结果:试验组治疗效果优于对照组[OR=4.06,95%CI(2.68,6.15)];治疗后试验组房颤改善情况优于对照组[OR=4.63,95%CI(2.80,7.64)];治疗后试验组左室射血分数(LVEF)高于对照组[MD=5.17,95%CI(3.08,7.26)];治疗后静息心率低于对照组[MD=-9.86,95%CI(-17.88,-1.84)];稳心颗粒联合美托洛尔治疗非瓣膜性房颤的不良反应少,安全性好。结论:稳心颗粒联合美托洛尔治疗非瓣膜性房颤疗效优于单纯西药治疗,且安全性好,临床上值得进一步推广使用。
Objective:To evaluate the efficacy and safety of the Wenxin granule(稳心颗粒)plus metoprolol on non-valvular atrial fibrillation.Methods:The RCT literature of treating non-valvular atrial fibrillation with the Wenxin granule before May 2017 was collected,according to computer retrieval and manual retrieval of theme words with free words in CNKI,Wanfang Data,VIP,PubMed,Embase,Cochrane and others.The RCT literatures were chosen according to the inclusion criteria.Then,the quality of included literature were evaluated by Jadad scale and Cochrane collaboration bias risk assessment tools.According to raw data from the literature,the efficacy was evaluated.1938 patients of 24 studies were included.Results:The efficacy in the treatment group was better than the control group[OR=4.06,95%CI(2.68,6.15)];the atrial fibrillation in the experimental group was better than the control group[OR=4.63,95%CI(2.80,7.64)];the LVEF in the experimental group was higher than the control group[MD=5.17,95%CI(3.08,7.26)];the resting heart rate in the experimental group was lower than the control group[MD=-9.86,95%CI(-17.88,-1.84)].The Wenxin granule plus metoprolol on nonvalvular atrial fibrillation shows a few adverse reactions,is good safety.Conclusion:The Wenxin granule plus metoprolol is superior to western medicine on treating non-valvular atrial fibrillation,is safe and worthy of promotion.
