摘要
目的:探讨妊娠早期亚临床甲状腺功能减退症与妊娠结局及新生儿并发症的相关性。方法:2015年1月~2016年1月,妊娠早期亚临床甲状腺功能减退症60例,包括甲减病例20例,亚甲减病例40例,同时选择同期产检健康、妊娠早期孕妇作为对照组40例,使用全自动电化学发光分析系统和血清甲状腺激素试剂盒对患者进行检测。结果:(1)与健康对照组对比,甲减和亚甲减患者TSH明显更高,T3、T4、FT3、FT4明显更低,P<0.05;和亚甲减对比,甲减患者TSH明显更高,T3、T4、FT3、FT4明显更低,P<0.05;(2)与健康对照组对比,观察组患者不良妊娠结局发生率、新生儿并发症发生率明显更高,P<0.05;在观察组中接受治疗患者不良妊娠结局发生率、新生儿并发症发生率低于未接受治疗患者,P<0.05。结论:妊娠早期亚临床甲状腺功能减退症与妊娠结局及新生儿并症密切相关,妊娠早期亚临床甲状腺功能减退症是导致不良妊娠结局和新生儿并发症的重要原因。
Objective:To investigate the relationship between hypothyroidism in early pregnancy and pregnancy outcome and neonatal complications.Methods:60 cases of patients with hypothyroidism treated in a hospital from January 2015 to January 2016 including 20 cases of hypothyroidism and 40 cases of subclinical hypothyroidism were selected.At the same time,40 cases of healthy pregnant women were selected as the control group.Patients were detected by automatic electrochemiluminescence analysis system and serum thyroid hormone kit.Results:(1)TSH in hypothyroidism and subclinical hypothyroidism patients was significantly higher than that in healthy control group,and the T3,T4,FT3,T4 were significantly lower(P<0.05).Compared with subclinical hypothyroidism,the TSH of hypothyroidism patients was significantly higher than that of hypothyroidism patients,and the T3,T4,FT3,T4 were significantly lower(P<0.05).(2)Compared with the healthy control group,The incidence of adverse pregnancy outcomes and neonatal complications in the observation group was significantly higher(P<0.05).The incidence of adverse pregnancy outcomes and neonatal complications of treated patients in the observation group was lower than that of untreated patients(P<0.05).Conclusion:Early pregnancy subclinical hypothyroidism is closely related with the pregnancy outcome and neonatal complications,and the early pregnancy subclinical hypothyroidism is an important cause of adverse pregnancy outcomes and neonatal complications.
