摘要
目的探讨早期梅毒血清学诊断和治疗效果评估的临床价值。方法选择2015年1月—2017年1月于我院诊治的早期梅毒患者,共纳入95例患者。检测按照治疗前梅毒甲苯胺红不加热血清反应素试验(TRUST)滴度分为高滴度组和低滴度组。比较两组患者治疗3个月、6个月、12个月的滴度变化。记录治疗前、治疗3个月、6个月、12个月梅毒螺旋体特异性抗体明胶凝集试验(TPPA)阳性结果。结果治疗前、治疗3个月、6个月、12个月95例患者TRRA检测结果均为阳性。治疗前TRUST滴度为阴性3例,1∶1~1∶4者40例,纳入低滴度组;≥1∶8者52例,纳入高滴度组。治疗3个月、6个月、12个月,低滴度组患者的临床效果优于高滴度组,差异有统计学意义(P<0.05)。结论 TRRA联合TRUST可提高早期梅毒的诊断,TRUST可监测梅毒患者的治疗效果,低滴度患者治疗后转归优于高滴度患者。
Objective To explore the clinical value of the early diagnosis and treatment of syphilis for syphilis.Methods 95 patients with early syphilis were selected from January 2015 to January 2017 in our hospital.The test was divided into high titer group and low titer group according to the Unheated sera reactivity test of toluidine red in syphilis(TRUST)titer before treatment.The changes in the titer of the two groups were compared for 3 months,6 months and 12 months.Gelatin agglutination test of treponema pallidum specific antibody(TPPA)positive results were recorded before and after treatment for 3 months,6 months and 12 months.Results Before treatment,3 months,6 months,and 12 months,the results of TRRA were all positive in 95 patients.Before treatment,the titers of TRUST was negative in 3 cases,1:1~1:4 in 40 cases,into the low titer group;52 patients≥1:8,into the high titer group.The clinical effect of the low titer group was better than that of the high titer group for 3 months,6 months and 12 months,and the difference was statistically significant(P<0.05).Conclusion TRRA combined with TRUST can improve the diagnosis of early syphilis,TRUST can monitor the therapeutic effect of syphilis patients,and the prognosis of patients with low titer is better than that of patients with high titer.
