摘要
目的探讨引流器植入联合小梁切除术治疗难治性青光眼的临床疗效。方法选取2014年7月至2016年9月在我院行引流器(Ahmed)植入联合小梁切除术治疗的难治性青光眼患者30例(39眼)作为观察组,选取2012年1月至2014年6月于我院行小梁切除术的难治性青光眼患者30例(40眼)作为对照组。对两组患者的临床资料进行回顾性分析,观察两组患者手术前、术后眼压及视力情况,并统计随访期间眼部并发症发生率。结果与术前比较,术后1周及术后1个月两组患者眼压均较低,术后1个月两组眼压均较术后1周升高(P<0.05);术后1周及术后1个月,观察组眼压均明显低于对照组(P<0.05)。术后1周及术后3个月,两组患者视力均明显提高,且观察组视力水平明显高于对照组(P<0.05);观察组眼部并发症总发生率为10.26%,显著低于对照组的30.00%(P<0.05)。结论较小梁切除术,联合引流器植入对难治性青光眼患者眼压的控制效果更佳,对视力的改善也更显著,术后并发症少,临床应用价值高。
Objective To investigate the clinical effect of drainage device implantation combined with trabeculectomy in the treatment of refractory glaucoma.Methods From July 2014 to September 2016,30 refractory glaucoma patients(39 eyes)treated with drainage device implantation combined with trabeculectomy were selected as the observation group.From January 2 01 2 to June 2014,30 refractory glaucoma patients(40 eyes)treated with trabeculectomy were collected as the control group.The clinical data of the two groups were retrospectively analyzed.The intraocular pressure and visual acuity before and after operation and complications were compared between the two groups.Results Compared with before operation,the intraocular pressures at 1 week and 1 month after operation of the two groups decreased,and those at 1 month after operation of the two groups were higher than at 1 week after operation(P<0.05),at 1 week and 1 month after operation,the intraocular pressures in the observation group were lower than those of the control group(P<0.05).At 1 week and 1 month after operation,the visual acuity of the two groups improved,and those of the observation group were higher than the control group(P<0.05).The total incidence of complications in the observation group was 10.26%,which was lower than 30.0 0%of the control group(P<0.05).Conclusion Compared with trabeculectomy alone,drainage device implantation combined with trabeculectomy in the treatment of refractory glaucoma has better improvement of intraocular pressures and visual acuity,and it has less complications,which has high clinical application value.
