摘要
目的探析血清脂联素(APN)在2型糖尿病冠心病患者甲状腺功能减退时的临床意义。方法选取2014年1月至2015年11月在我院接受诊治的240例2型糖尿病患者作为研究对象,分为B组单纯2型糖尿病、C组2型糖尿病冠心病甲状腺功能正常、D组2型糖尿病冠心病合并甲状腺功能减退,各80例,另选取同期健康体检者80例作为对照组(A组)。对比4组血糖、甲状腺功能、血脂和血清脂联素水平情况。结果 A组糖化血红蛋白(Hb A1c)、空腹血糖(FBG)水平均低于B、C、D 3组(均P<0.05)。D组促甲状腺激素(TSH)水平高于A、B、C组(均P<0.05);C、D组游离三碘甲状腺原氨酸(FT3)水平低于A、B组(均P<0.05);4组游离甲状腺素(FT4)、总胆固醇(CHOL)水平比较,差异无统计学意义(均P>0.05)。A组高密度脂蛋白(HDL)、甘油三酯(TG水平高于B、C、D组,差异有统计学意义(P<0.05)。结论 APN水平与FT3、FT4、HDL呈正相关联系,与Hb A1c、FBG、TSH、TG呈负相关联系,APN水平通过下调可促使冠心病的发展,且APN为冠心病保护因子。
Objective To investigate clinical significance of adiponectin(APN)during hypofunction of thyroid in type 2 diabetes mellitus patients with coronary heart disease. Methods A total of 240 patients with type 2 diabetes mellitus treated in our hospital from January 2014 to November 2015 were selected as subjects.According to the difference of patients’basic diseases,group B,group C and group D were divided.Group B was simple type 2 diabetes,group C was type 2 diabetes mellitus with normal thyroid function,and group D was type 2 diabetes mellitus with coronary heart disease and hypothyroidism.Another 80 healthy people admitted to our hospital for the same period were selected as the control group(group A).Blood glucose,thyroid function,blood lipid and serum APN were observed and compared between the four groups. Results The levels of HbA1c and FBG in group A were lower than those in groups B,C and D(all P<0.05).The levels of thyroid stimulating hormone(TSH)in group D were higher than those in groups A,B and C(all P<0.05).The levels of free triiodothyronine(FT3)in groups C and D were lower than those in groups A and B(all P<0.05).Free thyroxine(FT4)and total cholesterol(CHOL)levels were not statistically significantdifference in four groups(all P>0.05).High-density lipoprotein(HDL),triglyceride(TG)level of A group were higher than B,C,D group,and the difference was statistically significant(P<0.05). Conclusion Serum APN level was positively correlated with FT3,FT4,HDL,and negatively correlated with HbA1c,FBG,TSH,TG.The down-regulation of APN level could promote the development of coronary heart disease,and APN was the coronary heart disease protection factor.
