摘要
目的:了解长期卧床合并下呼吸道感染患者耐药菌的分布及耐药特点。方法:选择150例长期卧床并发下呼吸道感染的老年患者,对其痰液标本中的细菌进行分类和鉴定,并利用纸片扩散法检测其耐药性。结果:痰液标本中共分离出136株菌株,菌株分布以革兰阴性杆菌为主,约占62.5%,主要为大肠埃希菌、肺炎克雷伯菌、铜绿假单胞菌、鲍氏不动杆菌;革兰阳性球菌以溶血性链球菌、肺炎链球菌和金黄色葡萄球菌为主。细菌耐药检测显示,大肠埃希菌、肺炎克雷伯菌和铜绿假单胞菌对β-内酰胺类抗生素耐药率大都>50%,但对含β-内酰胺酶抑制剂的抗生素和碳青霉烯类抗生素较敏感,而鲍氏不动杆菌耐药十分严重;革兰阳性球菌耐药也较严重,但对四代头孢、替考拉宁及万古霉素,仍较敏感。结论:长期卧床下呼吸道感染患者痰标本中分离出的细菌以革兰阴性杆菌为主,耐药检测结果显示该类患者耐药较严重。
Objective:To study the distribution and drug resistance of bacilli from sputum of bed-ridden patients with lower respiratory tract infection.Methods:The bacilli from sputum of 150 bed-ridden elderly patients with lower respiratory tract infection were isolated and identified,disk diffusion test was applied to evaluate the antibiotic resistance.Results:136 strains were isolated from sputum,of which Gram-negative bacilli take up a great proportion(62.5%).The common bacilli were K.pneumonia,E.coli,P.aeruginosa,A.baumannii,S.pneumonia,H.streptococcus and S.aureus.K.pneumonia,E.coli,and P.aeruginosa had the high resistance toβ-lactam antibiotics,but sensitive toβ-lactamase inhibitor compounds and carbapenems.Multiple-antibiotic resistance of A.baumannii appeared greatly.Many gram-positive strains developed multi-drug resistance to the commonly used antibiotics,but were still sensitive to cefepine,teicoplanin and vancomycin.Conclusions:Gram-negative bacilli take up a great proportion in the bacilli bacilli from sputum of bed-ridden patients with lower respiratory tract infection,and the drug resistance rates was severe.
