摘要
近几年有关危重症营养支持研究,尤其是大样本随机对照试验(RCT)研究,使人们对应激与炎症反应期间的代谢特征与营养需要变化的了解日渐深入,对于危重疾病早期营养供给的认识也得到了更新,尤其是早期营养供给量对预后可能产生的影响更加明确。研究证实高营养风险患者早期积极的营养补充对预后产生有益影响。早期适当降低热卡供给以及充分的蛋白质供给可能更符合此时的生理与代谢改变及预后改善。胃肠道喂养不耐受是早期肠内营养面临的困难,识别风险、加强评估、流程管理等措施是其有效实施的必要保障。
Recently,numerous of important studies,especially large randomized control trials about nutrition support in critically ill patients promoted a better understanding of the energy and protein needs during the acute stress and informatory phase of the disease,as well as the effect of early nutrition dosage on the outcome.It was demonstrated that early nutrition could improve the outcome in patients with high nutrition risk.Proper early hypocaloric and sufficient protein feeding may be more in line with the physiological and metabolic changes and improve the outcome.Gastrointestinal intolerance is the most common challenge in early enteral nutrition which demand risk identify and assessment and protocol management.
