超声引导下连续股神经阻滞在全膝关节置换术术后镇痛中的效果观察 被引量:2

摘要 目的:探讨超声引导下连续股神经阻滞在全膝关节置换术术后镇痛中的效果。方法:选取2014年1月至2016年12月在我院行全膝关节置换术的患者80例,随机分为观察组(超声引导下连续股神经阻滞)和对照组(静脉自控镇痛)各40例。对比两组临床疗效。结果:两组术后8hVAS评分比较,差异无统计学意义(P>0.05)。观察组术后24h、48h和72hVAS评分显著低于对照组(P<0.05)。两组术后12h患肢主动关节屈曲角度比较,差异无统计学意义(P>0.05)。观察组术后24h和术后48h患肢主动关节屈曲角度均大于对照组(P<0.05)。观察组不良反应发生率显著低于对照组(P<0.05)。结论:全膝关节置换术术后采用超声引导下连续股神经阻滞镇痛效果良好,值得临床推广。 Objective: To investigate the effect of continuous femoral nerve block guided by ultrasound in postoperative analgesiaafter total knee arthroplasty. Methods: 80 patients with total knee replacement in our hospital from January 2014 to December2016 were randomly divided mto observation group (ultrasound-guided continuous femoral nerve block) and control group (intravenousanalgesia) ,each groupwere 40 patents. The clinical efficacy of the two groups was compared. Results : There wasno significant (difference m 8h VAS score between the two groups (P > 0. 05) . The scores of 24h, 48h and 72h VAS m the observationgroup were significantly lower than those m the control group (_P < 0. 05) . the two groups in the flexion angle of active joints of 12h patients after operation (P > 0. 05) . The flexion angle of theactive joints of patients postoperative the 24h and 48h in the observation group was significantly higher than those in the controlgroup (P < 0. 05) . The nicidence of adverse reactions n the observation group was significantly lower than n the controlgroup (P < 0. 05) . Conclusion: The effett of ultrasound-guided continuous femoral nerve block analgesia after total knee arthroplastyis good, which is worthy of clinical promotion.
