摘要
目的采用系统评价的方法了解我国儿童青少年因跌伤死亡的发生现况。方法从CNKI、万方、CBM、VIP等中文数据库及PubMed、OVID、ScienceDirect外文数据库，系统检索2002—2012年我国0~19岁儿童青少年跌落伤死亡的文献，按照纳入/排除标准筛选文献，提取相关信息。结果共检索出符合标准的跌落伤死亡率文献31篇。儿童青少年跌伤合并死亡率为1.44/10万；男童跌伤死亡率（1.43/10万）高于女童（0.84/10万），差异有统计学意义（P<0.05);东部、中部、西部地区儿童跌伤死亡率分别是1.12/10万、1.98/10万、4.36/10万，西部地区高于东、中部地区，差异有统计学意义（P<0.05），敏感性分析表明本次Meta分析结果稳定，相对可靠。结论在我国西部等经济欠发达地区，儿童青少年因跌落伤而造成的死亡水平较高，应加强这些地区的跌落伤预防干预及医疗救助。
Objective Using the method of Meta-analysis to comprehensively evaluate unintentional fall mortality among children and adolescents in China.Methods With established inclusion criteria,studies searched the following databases:CNKI database,Wangfang database,CBM database,VIP database,PubMed database,OVID medical full-text database,and Science Direct foreign language database.All data were collected from the literature published during2002-2012about unintentional injury of children and adolescent aged0~19years old evaluation sensitivity and bias of published documents were conducted and the injury incidence was analyzed considering the factors of gender,regional,geographical,injury diagnosis standard subgroup after extracting information.Results Across31studies,the total unintentional fall mortality66after merged was1.44/10.Subgroup analyses identified that the unintentional fall mortality of boys(1.43/10)higher than girls(0.84/106,P<0.05).The mortality of children fall in the Eastern,Central,and Western regions were1.12/105,1.98/10,4.36/105,respectively.The highest unintentional fall mortality was in the west area4.36/10,a significant difference in unintentional fall mortality existed across different geographical areas.(χ2=491.87,P<0.05).Sensitivity analysis showed that the Meta analysis result is stable and reliable relatively.Conclusion At present,the unintentional fall mortality among children and adolescents was higher;the underdeveloped economy western region was a key factor influencing the unintentional fall prevention and medical assistance.
