摘要
目的探究某市2型糖尿病(T2DM)患者一级亲属中糖尿病及糖尿病前期的患病情况及影响因素。方法在某市本地区常住人口中选取230例T2DM患者的1206位一级亲属进行横断面调查,糖尿病及糖尿病前期诊断采用1999年WHO糖尿病和高血糖的诊断标准。结果 1206例T2DM患者一级亲属中,男性751例,女性455例,平均年龄(46.36±13.09)岁。T2DM总体患病率为10.94%,男性患病率6.79%,女性患病率17.80%;T2DM前期患病率为15.00%,男性患病率9.73%,女性患病率23.36%;T2DM及T2DM前期患病率在各年龄组中存在明显差异性,随着年龄的增长,T2DM及T2DM前期患病率均逐渐上升(P<0.001);T2DM及T2DM前期患病率在TG(≥1.7 mmol/L)、BMI(>25 kg/m2)、腰围(≥80 cm)因素比较中存在明显差异(P<0.001);年龄、TG水平是影响T2DM发生的独立危险因素(P<0.001);年龄、BMI是影响T2DM前期发生的独立危险因素(P<0.001)。结论某市T2DM患者一级亲属中糖尿病及糖尿病前期患病率较高,需采取有效措施干预糖代谢异常的流行。
Objective To investigate the prevalence of diabetes mellitus and pre-diabetes mellitus in first-degree relatives of type2diabetes mellitus(T2DM)in a city.Methods1206first-degree relatives of230patients with T2DM in the resident population of a city were selected for a cross-sectional investigation.Patients with diabetes were chosen as the control group(n=693)while those without diabetes served as the study group(n=94).Waist circumference,blood pressure,blood lipid and anhydrous glucose tolerance test(OGTT)results in the two groups of patients were sorted out.Results Among the1206first-degree relatives,751were males and455were females,with an average age of(46.36+13.09)years.The overall prevalence of T2DM was10.94%(6.79%for males and17.80%for females).The prevalence of prediabetes was15%(9.73%for males and23.36%for females).The prevalence of T2DM and prediabetes was significantly different between different age groups and tended to increase with the age of patients(P<0.001T2DM and T2DM).TG(1.7mmol/L),BMI(>25kg/m2)and waist circumference(more than80cm)had much effect on the occurrence of T2DM and prediabetes(P<0.001).Age and TG levels were independent risk factors that influenced the occurrence of T2DM(P<0.001)while age and BMI were independent risk factors for prediabetes(P<0.001).Conclusion The prevalence of diabetes and prediabetes is higher in T2DM relatives of patients in this city,and effective measures should be taken to control abnormal glucose metabolism.
