摘要
目的研究下颌第三磨牙导致下颌第二磨牙远中邻面龋坏的相关影响因素。方法收集因下颌第三磨牙不适而就诊的患者250例,运用锥形束CT分析下颌第二磨牙远中邻面龋的患病率。探究年龄、下颌第三磨牙阻生角度及深度、第二和第三磨牙接触位点与下颌第二磨牙远中邻面龋坏的相关性。结果下颌第二磨牙的远中邻面龋的患病率为52%。>38岁年龄组的第二磨牙远中邻面龋坏发生率明显高于其他组别(P<0.05);下颌第三磨牙阻生角度为40°~70°时,第二磨牙远中邻面龋坏发生率最高(P<0.05);下颌第三磨牙与第二磨牙接触点位于釉牙骨质界处第二磨牙龋坏的发生率高于其他组别(P<0.05)。结论患者年龄较大时,下颌第三磨牙阻生角度为40°~70°且接触点位于釉牙骨质界时,第二磨牙远中更易发生龋坏,建议尽早拔除阻生齿。
ObjectiveTo analyze the related factors of the distal interproximal caries in the mandibular second molar caused by the compacted mandibular third molar.MethodsTwo hundred fifty patients who had eruption problems with their third molars were included in this study.Cone beam computed tomography(CBCT)images were used to analyze the prevalence of the distal interproximal caries in the mandibular second molar.The relations of distal interproximal caries in the mandibular second molar with age of patients,the impacted angulation and position of the third molars(Pell and Gregory classification),the contract point between the second molar and third molar were analyzed.ResultsThe prevalence of the distal interproximal caries in the mandibular second molar was52%.The prevalence of distal interproximal caries in the mandibular second molar in age group>38years was significantly higher than that of other groups(P<005).With the impacted angulation between40°-70°,the prevalence was the highest(P<0.05);and whith the contract point of the second molar and third molar situated in the cemento enamel junction(CEJ),the caries were more frequently(P<0.05).ConclusionWith the increasing age,the impacted angulation40°70°,and the contract point situated in the cemento enamel junction(CEJ),the distal interproximal caries in the mandibular second molar will be more serious.These impacted third molars should be extracted as early as possible.
