摘要
目的探讨腹腔镜完全腹膜外疝修补术(TEP)对中青年男性腹股沟疝患者性功能的影响。方法纳入于2014年1月-2016年2月接受TEP的腹股沟疝患者147例为观察组,纳入同期接受腹腔镜经腹腹膜前疝修补术(TAPP)的腹股沟疝患者133例为对照组,开展回顾性分析。对比两组手术效果、性功能的变化。结果观察组手术时间、住院时间、术后疼痛时间明显短于对照组,住院费用少于对照组,差异有统计学意义(P<0.05);观察组术后精液α-葡萄糖苷酶(α-Glu)、果糖(Fru)和酸性磷酸酶(ACP)水平均明显高于对照组,差异有统计学意义(P<0.05)。两组术后均出现少量并发症,经对症处理后均痊愈,两组并发症发生率及复发率对比,差异无统计学意义(P>0.05)。结论相较于腹腔镜TAPP,对腹股沟疝患者采用腹腔镜TEP术式可以进一步缩短手术时间及术后恢复时间,且不增加并发症及复发率,同时还有助于促进患者精液质量的恢复,值得关注。
Objective To investigate the effect of laparoscopic totally extraperitoneal(TEP)on sexual function in young and middle-aged patients with inguinal hernia.Methods147male with inguinal hernia treated by TEP from January2014to Feberary2016were included in the observation group,while133male with inguinal hernia treated by laparoscopic transabdominal preperitoneal(TAPP)during the same time were included in the control group.The effect of surgery and changes on sexual function between the two groups were retrospective analyzed and compared.Results The operative time,length of hospital stay,time of postoperative pain and in the observation group shorter than in the control group.The differences between the two groups were statistically significant(P<0.05).The level of seminal alpha glucosidase,fructose contents and acid phosphatase in the observation group were higher than in the control group at12months after surgery.The differences between the two groups were statistically significant(P<0.05).There were a small number of complications in both groups.After symptomatic treatment,all complications were cured.The ratio of complications and recurrences of the two groups have no significantly differences(P>0.05).Conclusion Compared with laparoscopic TAPP,laparoscopic TEP can shorten the time of postoperative recovery,and will not increase the complications and recurrences.It is good for recovery of semen quality.It is worthy of attention.
