摘要
目的分析老年高血压合并急性脑梗死患者发生呼吸机相关性肺炎(ventilator associated pneumonia,VAP)的影响因素。方法回顾性分析2012年6月至2017年2月本院收治的因老年高血压合并急性脑梗死后行气管插管进行机械通气救治的140例患者的临床资料,分析老年高血压合并急性脑梗死患者发生VAP的影响因素。结果140例老年高血压合并急性脑梗死患者给予机械通气后,共有43例(30.7%)患者发生VAP。年龄>70岁、机械通气时间≥3天、体质指数>28 kg/m^2、气管切开及服用抑酸药物是导致老年高血压合并急性脑梗死患者发生VAP的独立危险因素(P<0.05)。结论老年高血压合并急性脑梗死患者VAP的发生与多种危险因素相关,针对这些高危因素必须合理控制患者饮食、减轻体重、缩短机械通气时间、严格控制抑酸药物的使用剂量及积极治疗基础疾病,以期有效预防和减少老年高血压合并急性脑梗死患者发生VAP。
Objective To analyze the influencing factors of ventilator associated pneumonia(VAP)in elderly hypertensive patients complicated with acute cerebral infarction.Method Retrospectively analyzed the clinical data of140cases of elderly hypertensive patients complicated with acute cerebral infarction treated by mechanical ventilation after tracheal intubation from June2012to February2017in our hospital.Analyzed the influencing factors of VAP in elderly hypertensive patients complicated with acute cerebral infarction.Result After mechanical ventilation was given to140elderly hypertensive patients complicated with acute cerebral infarction,a total of43patients(30.7%)developed VAP.Age>70years old,mechanical ventilation≥3days,body mass index>28kg/m2,tracheotomy and taking acid-suppressing drugs were the independent risk factors for VAP in elderly hypertensive patients complicated with acute cerebral infarction(P<0.05).Conclusion VAP is associated with multiple risk factors in elderly hypertensive patients complicated with acute cerebral infarction,in view of these high risk factors,we must rationally control the diet,reduce weight,shorten the time of mechanical ventilation,strictly control the dosage of antacid drugs and actively treat the related basic diseases of patients,to effectively prevent and reduce the occurrence of VAP in elderly hypertensive patients complicated with acute cerebral infarction.
