超声对胎儿动脉导管异常与先天性心脏病及预后相关研究 被引量:3

摘要 目的超声检测胎儿动脉导管异常,分析其与先天性心脏病的关系及预后。方法 1 098例孕妇在本院行产前超声心动图检查,重点观察胎儿动脉导管异常及与先天性心脏病发生情况,总结胎儿动脉导管异常的超声表现及分类,分析不同类型动脉导管异常与胎儿先天性心脏病的关系。结果 1 098例胎儿共检出动脉导管异常57例,占5.2%,动脉导管形态异常15例,2例伴有先天性心脏病;动脉导管内径异常15例,9例合并先天性心脏病;动脉导管走形异常18例及动脉导管血流异常10例均合并先天性心脏病。57例动脉导管异常胎儿共合并先天性心脏病39例,占68.4%(39/57),各型所占比例分别为3.5%(2/57)、15.8%(9/57)、31.6%(18/57)、17.5%(10/57),动脉导管异常中以动脉导管走行异常和动脉导管血流异常合并先天性心脏病比例高,其次为动脉导管内径异常,与动脉导管形态异常差异有统计学意义(P<0.05)。结论不同类型动脉导管异常与胎儿先天性心脏病有一定关系,动脉导管走形异常及动脉导管血流异常对诊断胎儿先天性心脏病有重要价值。 Objective To analyze the correlation of fetal arterial duct abnormity with congenital heart disease and clinical prognosis based onthe ultrasonic cardiogram.Methods1098gravida of adopting the ultrasonic cardiogram in our hospital were selected.The correlation of fetal arterial duct abnormity with congenital heart disease was observed;according to the ultrasound manifestation and classification,the correlation of different types of arterial duct abnormity with congenital heart disease was analyzed.Results There were57cases with arterial duct abnormity(5.2%).For15cases with abnormal shapes of arterial ducts,there were2cases complicated with congenital heart disease.For15cases with abnormal inner diameter of arterial ducts,there were9cases complicated with congenital heart disease.For18cases with abnormal directions of arterial ducts and abnormal blood flow of arterial ducts,there were10cases complicated with congenital heart disease.To sum up,57cases with arterial duct abnormity had39cases complicated with congenital heart disease(68.4%,39/57).The incidence rate of congenital heart disease for each type of arterial duct abnormity was3.5%(2/57),15.8%(9/57),31.6%(18/57)and17.5%(10/57).For patients with abnormal directions of arterial ducts and abnormal blood flow of arterial ducts,the incidence rate of congenital heart disease was higher,followed by the patients with abnormal inner diameter of arterial ducts.It was significantly different from the patients with abnormal shapes of arterial ducts(P<0.05).Conclusion The different types of arterial duct abnormity is correlated with incidence rate of congenital heart disease.The detection of abnormal directions of arterial ducts and abnormal blood flow of arterial ducts is of great importance in the diagnosis of congenital heart disease.
