摘要
目的甲状腺切除术不同术式的甲状旁腺损伤程度尚存在争议,如何保护手术过程中的甲状旁腺功能对患者预后有重要意义。方法研究者回顾性纳入了1 264例首次接受甲状腺切除术治疗患者,并汇总了其临床资料,评估了全切组、双次组、一侧+对侧组、全切+清扫组四种手术方式中的甲状旁腺损伤事件(低血钙、症状性低钙血症及低PTH),并分析了术中甲状腺保护方式的重要性。结果全甲状腺切除术+中央淋巴结清扫术具有最高的甲状旁腺功能低下发生率(P<0.05),而术中纳米碳使用及传统甲状旁腺识别对于保护甲状旁腺具有重要的临床意义。结论甲状腺切除术手术范围越大,甲状旁腺功能约容易受到影响,术中保护甲状旁腺对于预防甲状旁腺功能低下有积极意义。
Objective To investigate the degree of parathyroid injury in different surgical procedures of thyroidectomy,and how to protect theprognosis of patients with thyroid function during operation.Methods The investigators retrospectively included1264patients who underwent thyroidectomyfor the first time and summarized their clinical data.Four surgical procedures were performed in all-cut group,double-group,lateral+contralateral group,total resection+(Hypocalcemia,symptomatic hypocalcemia and low PTH),and analyzed the importance of intraoperative thyroidprotection.Results Total thyroidectomy and central lymphadenectomy had the highest incidence of hypoparathyroidism(P<0.05),and intraoperativeuse of nano-carbon and traditional parathyroid gland identification for the protection of parathyroid gland has important clinical significance.Conclusion The greater the scope of thyroidectomy surgery,parathyroid function is likely to be affected,intraoperative protection of parathyroidgland for the prevention of hypopyroidism has a positive significance.
