摘要
目的探讨术前N末端脑钠肽前体(NT-proBNP)、左房容积(LAV)与心房颤动(房颤)射频消融术后复发的关系。方法选取2013年1月~2015年6月于郑州大学第一附属医院实施射频消融手术治疗的房颤患者179例,根据术后随访结果分为复发组76例、非复发组103例,比较两组一般资料、术前血浆NT-proBNP及LAV等指标,并采用非条件Logistic回归模型探讨血浆NT-proBNP及LAV对术后复发的影响。结果复发组的NT-proBNP、LAV、左房射血分数(LAEF)低于非复发组,差异具有统计学意义(P<0.05);复发组的左室射血分数(LVEF)与非复发组比较差异不具有统计学意义(P>0.05);NT-proBNP、LAV升高是房颤患者射频消融术后复发的危险因素(OR=1.852,95%CI:1.378~5.033;OR=1.367,95%CI:1.106~2.954,P<0.05),LAEF升高是房颤患者射频消融术后复发的保护因素(OR=0.629,95%CI:0.351~0.878,P<0.05)。结论 NT-proBNP、LAV升高是房颤患者射频消融术后复发的独立危险因素。
Objective To investigate the relationship between preoperative N-terminal pro brain natriureticpeptide(NT-proBNP),left atrial volume(LAV)and recurrence of atrial fibrillation after radiofrequency catheterablation.Methods From January2013to June2015,179atrial fibrillation patients received radiofrequencyablation surgery were divided into relapse group with76cases and non-recurrent group with103cases based onfollow-up result.We compared preoperative plasma NT-proBNP and LAV index between two groups,and used thenon-conditional Logistic regression model to explore the influence of LAV on plasma NT-proBNP on the recurrenceafter operation.Results NT-proBNP,LAV,LAEF was lower in the recurrence group when compared with thenon-recurrence group(P<0.05).There was no significant difference in the left ventricular ejection fraction(LVEF)between the recurrence group and the non-recurrence group(P>0.05).NT-proBNP increasing and LAV increasingare risk factors of recurrence after radiofrequency ablation.Elevated LAEF is a protective factor for recurrence ofatrial fibrillation after radiofrequency ablation Conclusion NT-proBNP,LAV is an independent risk factor forrecurrence of atrial fibrillation after radiofrequency catheter ablation.
