摘要
目的探讨宫内生长受限(IUGR)对早产儿脑功能发育的影响。方法采用Nicolet One脑功能监护仪对2015年1月至2016年2月住院、胎龄32~36周的110例早产儿,其中小于胎龄儿(SGA)50例、适于胎龄儿(AGA)60例,于生后72小时内进行监测,每次连续监测4~6小时,比较两组早产儿脑电波形的连续性、睡眠-觉醒周期、爆发间期、最低和最高电压等指标的差异。结果 SGA组振幅整合脑电图(a EEG)连续性脑电图及睡眠-觉醒周期的出现率及最高、最低电压值均低于AGA组,爆发间期则长于AGA组,差异均有统计学意义(P<0.05)。在SGA组中,出生体质量<P_3组、P_3~P_5组、P_5~P_(10)组之间aEEG的连续性脑电图及睡眠-觉醒周期的出现率、爆发间期、最低和最高电压差异均无统计学意义(P>0.05)。结论 SGA早产儿连续性脑电图及睡眠-觉醒周期性出现率减少,爆发间期相对延长,最高及最低电压降低,提示IUGR一定程度上延迟或抑制了早产儿脑功能的发育。
Objective To explore the effect of intrauterine growth restriction(IUGR)on the development of brain function in premature infants.Methods A total of110premature infants of gestation age of32-36weeks were monitored by NicoletOne neonatal cerebral function monitor within72hours after birth during January2015to February2016.There were50small for gestational age infants(SGA)and60appropriate for gestational age infants(AGA).They were continuously monitored for4-6hours every time,and the indices of aEEG continuity,sleep wake cycle(SWC),inter-burst interval(IBI),minimum and maximum voltage were compared between two groups.Results The frequency of aEEG continuity,the rate of SWC,and the maximum and minimum voltage in SGA group were all lower than those in AGA group,while the IBI was longer than that in AGA group,and there were significant differences(P<0.05).The frequency of aEEG continuity,the rate of SWC,IBI,and the minimum and maximum voltage were similar(all P>0.05)among birth weight<P3group,P3~P5group and P5~P10group in SGA children.Conclusion SGA premature infants have a decreased rate of aEEG continuity and SWC,longer IBI,and lower maximum and minimum voltage.It is suggested that IUGR inhibits or delays the brain function development of premature infants to a certain extent.
