摘要
目的糖尿病伴早期肾病患者在控制血糖波动后，检测尿白蛋白排泄率及尿白蛋白/肌酐比值，观察血糖漂移程度增高是否为糖尿病早期肾病微血管病变程度的危险因子。方法3组受试人群：分别将血糖控制在慢性间歇性高血糖、慢性持续性高血糖、理想范围内。在不同时间段测量尿白蛋白排泄率及尿白蛋白/肌酐比值。结果3组患者的尿蛋白/肌酐检测：入组前：A组：134.1±57.3mg/g，B组：134.9±58.0mg/g，C组：135.8±50.6mg/g。20天后：A组：127.7±58.4mg/g，B组：60.6±23.9mg/g，C组：69.9±31mg/g.。6月后：A组：139.7±63.4mg/g，B组：145.5±49.5mg/g，C组：31.6±18.4mg/g。12月后：A组：161.4±74.3mg/g，B组：184.0±48.9mg/g，C组：14.2±6.7mg/g。结论血糖波动度过大与尿白蛋白排泄率及尿白蛋白/肌酐比值显著相关，血糖波动越大，漂移程度越高，糖尿病早期肾病进展风险越大。
Objective:Diabetes with early nephropathy patients in the control of blood glucose fluctuation,urinary albumin excretion rate and urinary albumin/creatinine ratio,observe the blood glucose excursion degree increasedwhether the early diabetic nephropathy risk factors.Methods:The participants were divided into three groupsrespectively:blood glucose control in chronic intermittent high glucose,chronicpersistent hyperglycemia,an ideal range.Measure urinary albumin excretion rate andurinary albumin/creatinine ratio in different time.Results:The results analysed by SPSS180software:The urine protein/creatinine of three groups:before entering the group:group A:134.1+57.3mg/g,groupB:134.9+58.0mg/g,groupC:135.8+50.6mg/g;20days later:group A:127.7+58.4mg/g,group B:60.6+23.9mg/g,groupC:69.9+31.0mg/g;six month:groupA:139.7+63.4mg/g,group B:145.5+49.5mg/g,group C:31.6+18.4mg/g;twelve month:group A:161.4+74.3mg/g,group B:184+48.9mg/g,group C:14.2+6.7mg/g.Conclusions:Blood glucose fluctuation was significantly related to urinary albumin excretion rate and urinary albumin/creatinine ratio,the higher drift degree of the blood glucose,the greaterrisk of early diabetic nephropathy progression.
