64排螺旋CT肝脏灌注对血吸虫性肝硬化分级及肝储备功能的研究 被引量:3

摘要 目的:探讨64排多层螺旋CT肝脏灌注成像在评估血吸虫肝硬化程度和肝脏储备功能中的临床应用价值。方法:64排螺旋CT肝脏灌注成像88例,其中轻度、中度、重度血吸虫性肝硬化分别为23例、25例和27例,健康志愿者及因其他疾病行腹部CT检查者13例作为对照组,测量各组的肝脏灌注参数,研究各组不同的肝脏灌注参数差异、CT形态学分级和Child-Pugh功能分级之间的关系。结果:3组血吸虫性肝硬化组的血容量(BV)、肝血流量(BF)、对比剂平均通过时间(MTT)、肝动脉分数(HAF)、肝动脉灌注量(HAP)值间差异有统计学意义(P<0.05)。结论:64排多层螺旋CT灌注成像可以作为评估血吸虫肝硬化程度和肝脏储备功能的重要参考。 Objective:To investigate the clinical application value of64slice spiral CT liver perfusion on the assessment of theclassification of hepatic cirrhosis due to schistosomiasis and liver reserve function.Methods:64slice spiral CT liver perfusionimaging in88cases,among them,mild,moderate and severe schistosomiasis cirrhosis were23cases,25cases and27cases.Healthyvolunteers and13patients with other diseases underwent abdominal CT examination were selected as the control group.Liverperfusion parameters were measured in each group.We studied the relationship between different hepatic perfusion parameters,CTmorphological classification and Child-Pugh classification.Results:There was significant difference in blood volume(BV),hepaticblood flow(BF),mean time between contrast agent(MTT),hepatic artery fraction(HAF),hepatic artery perfusion(HAP)value in3groups of patients with schistosomiasis cirrhosis(P<0.05).Conclusion:64slice spiral CT perfusion imaging can be used as animportant reference for evaluating the degree of liver cirrhosis and liver function.
