摘要
目的 :探讨不同止血方式对腹腔镜卵巢巧克力囊肿剥除术患者卵巢功能的影响。方法 :将2014年7月~2015年7月本院收治的99例卵巢巧克力囊肿患者随机分成对照组、镜下缝合组、单极电凝组,每组患者各33例。对照组手术采用缝合止血,镜下缝合组采用镜下缝合止血、单极电凝组采用单极电凝止血。在患者手术前1d、手术后30d、手术后60d时,记录检测并观察患者卵巢功能、窦卵泡数变化情况。结果 :手术后30d、60d,LH、E2、FSH指标变化与手术前1天相比,差异具有统计学意义(P<0.05),在E2指标中,单极电凝组数值明显低于其他两组,差异具有统计学意义(P<0.05)。手术后30d,各组窦卵泡数较手术前均减低(P<0.05),同一时间点比较,单极电凝组明显低于其余两组,数据差异具有统计学意义(P>0.05)。结论:使用单极电凝止血法剥除囊肿对患者的卵巢功能的影响较小,后期卵巢功能恢复时间更短。
Objective:To investigate the effect of different methods of hemostasis on patients undergoing laparoscopic enucleation of ovarian chocolate cyst.Methods:In total,99patients with ovarian chocolate cysts were admitted and treated in our hospital from July2014to July2015.They were randomly divided into three groups(control group,laparoscopic suturing group,and monopolar electrocoagulation group)with33patients in each group,and they were treated with suture hemostasis,suture hemostasis under laparoscope,monopolar electrocoagulation hemostasis,respectively.The changes of ovarian and antral follicles were observed and recorded.Results:Comparing with preoperative one day,there were significantly changes in LH,E2,and FSH at30th and60th day after operation.The E2in monopolar electrocoagulation group was significant lower than that of other groups.After30days postoperatively,antral follicles in each group were significant less than before operation(P<0.05),however,it in monopolar electrocoagulation group was significant less than in other groups(P>0.05).Conclusions:Application of monopolar electrocoagulation has less damage for performing ovarian enucleation,and less time for ovaries to recovery.
