睡眠呼吸暂停低通气综合征对心房颤动风险影响的Meta分析 被引量:4

摘要 目的系统评价睡眠呼吸暂停低通气综合征是否增加普通人群心房颤动的发病风险。方法利用计算机检索Pub Med、Cochrane、Medline、CNKI、维普、万方等数据库中关于睡眠呼吸暂停低通气综合征与普通人群心房颤动风险之间关系的观察性研究,检索时间截止至2016年10月。根据所设定的文献纳入与排除标准,由2名评价员独立完成文献筛选、资料提取、质量评价,纳入符合标准的文献,再利用Rev Man 5.3软件进行Meta分析。结果初步检索共得到232篇文献,最终纳入7篇符合标准的文献。经Meta分析表明,睡眠呼吸暂停低通气综合征明显增加普通人群心房颤动的发病风险(RR=1.80,95%CI:1.41~2.29,P<0.00001,I2=69%);与正常组相比,中度睡眠呼吸暂停低通气综合征组的心房颤动发病风险增加111%(RR=2.11,95%CI:1.22~3.64,P=0.008,I^2=70%),重度睡眠呼吸暂停低通气综合征组的心房颤动发病风险增加164%(RR=2.64,95%CI:1.52~4.58,P=0.0005,I^2=72%)。结论睡眠呼吸暂停低通气综合征明显增加普通人群心房颤动的发病风险,并且随着呼吸暂停低通气指数的增加,心房颤动的发病风险也在逐级增加。 Objective To review systematically whether or not sleep apnea hypopnea syndrome(SAHS)increasing the risk of atrial fibrillation(AF)in common population.Methods The databases of PubMed,CochraneLibrary,MedLine,CNKI,VIP Database and WanFang Database were retrieved with computer for searching theobservational studies on the relationship between SAHS and AF risk in common population up to Oct.2016.Theliterature screening,data extracting and quality reviewing were completed by2researchers according to inclusionand exclusion criteria,and eligible documents were included and given a Meta-analysis by using RevMan5.3software.Results There were totally232documents retrieved and7eligible documents were intaken.The resultsof Meta-analysis showed that SAHS increased significantly AF risk in common population(RR=1.80,95%CI:1.41-2.29,P<0.00001,I2=69%).Compared with normal group,medium SAHS increased AF risk by111%(RR=2.11,95%CI:1.22-3.64,P=0.008,I2=70%)and severe SAHS increased AF risk by164%(RR=2.64,95%CI:1.52-4.58,P=0.0005,I2=72%)in SAHS group.Conclusion SAHS can significantly increase AF risk in commonpopulation,and as the index of SAHS increases,AF risk increases gradually.
