摘要
目的探讨分析2型糖尿病伴骨质疏松患者的相关影响因素,为进一步的临床治疗提供科学的依据。方法 184例2型糖尿病患者,根据患者的骨密度水平分为观察组(伴骨质疏松,102例)和对照组(未伴骨质疏松,82例)。分别调查两组患者的一般资料及检测相关生化指标,分析2型糖尿病伴骨质疏松患者相关影响因素。结果两组患者性别、年龄、血钙、血磷、碱性磷酸酶、空腹血糖、总胆固醇、甘油三酯、低密度脂蛋白胆固醇和高密度脂蛋白胆固醇比较差异无统计学意义(P>0.05);两组患者体质量指数、病程、肌酐、尿微量蛋白、糖化血红蛋白和25羟基维生素D比较差异具有统计学意义(P<0.05);经多因素Logistic逐步回归分析,体重指数、病程、糖化血红蛋白和25羟基准维生素D是2型糖尿病伴骨质疏松症的影响因素。结论 2型糖尿病患者伴有骨质疏松的影响因素包括体质量指数、病程、糖化血红蛋白和25羟基维生素D,通过合理的增加体重、加强锻炼、有效的控制血糖以及补充维生素D能够有效的降低2型糖尿病患者发生骨质疏松症。
Objective To investigate and analyze related influencing factors for type2diabetes mellituscomplicated with osteoporosis patients,and to provide scientific reference for further clinical treatment.MethodsA total of184patients with type2diabetes mellitus were divided by their bone mineral density into observationgroup(complicated with osteoporosis,102cases)and control group(without osteoporosis,82cases).Researchwas made on their general data and related biochemical indexes in examination.Related influencing factors fortype2diabetes mellitus complicated with osteoporosis patients were analyzed.Results There was no statisticallysignificant difference of gender,age,blood calcium,blood phosphate,alkaline phosphatase,fasting blood glucose,total cholesterol,triglyceride,low density lipoprotein cholesterin and high density lipoprotein cholesterin betweenthe two groups(P>0.05).The difference of body mass index,disease course,creatinine,microalbuminuria,glycosylated hemoglobin and25-hydroxy-vitamin-D had statistical significance between the two groups(P<0.05).Multivariate Logistic stepwise regression analysis showed body mass index,disease course,glycosylatedhemoglobin and25-hydroxy-vitamin-D as the influencing factors for type2diabetes mellitus complicated withosteoporosis.Conclusion Influencing factors for type2diabetes mellitus complicated with osteoporosis includebody mass index,disease course,glycosylated hemoglobin and25-hydroxy-vitamin-D.Occurrence of osteoporosisin type2diabetes mellitus patients can be effectively reduced by rational weight increase,training enhancement,effective blood glucose control and vitamin-D supplement.
