摘要
目的探讨胃肠间质肿瘤(GIST)的临床病理特征。方法选择2010年1月~2015年12月收治的40例胃肠间质肿瘤患者作为研究对象,所有患者均行免疫化学检测CD117、CD34、SMA、Desmin、S100蛋白表达,回顾性分析GIST患者的临床病理特征,并就其免疫化学检测结果展开分析。结果 40例GIST患者26例病灶位于胃、9例病灶位于小肠、3例病灶位于结直肠、2例病灶位于腹膜后。肿瘤主要有梭形细胞和上皮样细胞组成,梭形细胞形态一致,呈梭形,细胞质丰富、嗜酸性,类似平滑肌细胞。上皮样细胞体积较大、形态多样,呈多角形或卵圆形,细胞质空壳,微嗜酸性,细胞核多样,与核分裂数成正比。免疫组化阳性检出率分别为CD117 95%、CD34 72.5%、SMA 7.5%、Desmin 2.5%、S100 2.5%。结论胃肠间质肿瘤具有独立的临床特征,CD117检测具有较高的敏感性和特异性,通过CD34、SMA、Desmin、S100等检测方式能够辅助CD117的检测,进一步评估和诊断良性肿瘤情况。
Objective To investigate the clinical and pathological features of gastrointestinal stromal tumors( GIST). Methods 40 cases of gastrointestinal stromal tumor patients from January 2010 to December 2015 as the research object.All patients underwent CD117, CD34, SMA, expression of Desmin and S100 protein immunohistochemical detection, a retrospective analysis of clinical and pathological features of patients with GIST, and the immunohistochemical detection results analysis. Results Of the 40 patients with GIST, the lesions were located in the stomach, and the lesions were locatedin the small intestine in 9 cases, in which the lesions were located in the rectum in 3 cases, and the lesions were located in the peritoneal cavity in 2 cases.The main tumor with spindle cells and epithelioid cells, spindle cells in Xingtai, fusiform, abundant eosinophilic cytoplasm, similar to smooth muscle cells.Epithelioid cells with large volume, shape, polygonal or ovoid, slightly eosinophilic cytoplasm, nucleus shell, diversity, and nuclear fission is proportional to the number of. The positive rates of immunohistochemistry were CD117 95%, CD34 72.5%, SMA 7.5%, Desmin 2.5%, S100 2.5%. Conclusion The clinical features of gastrointestinal stromal tumor with independent, CD117 has higher detection sensitivity and specificity by CD34, SMA, Desmin, S100 detection method can detect the auxiliary CD117, further evaluation and diagnosis of benign tumor.
