摘要
目的:探讨恩替卡韦联合复方鳖甲软肝片对慢性乙型肝炎肝硬化失代偿期患者肝纤维化指标的影响。方法:将63例慢性乙型肝炎肝硬化失代偿期患者随机分为治疗组和对照组。两组均采用常规内科综合治疗,治疗组加用恩替卡韦片和复方鳖甲软肝片口服,对照组加用阿德福韦酯胶囊口服,比较两组治疗前后透明质酸(HA)、层粘连蛋白(LN)、Ⅲ型前胶原(PCⅢ)、Ⅳ型胶原(ⅣC)、肝硬度(LSM)、肝门静脉直径和脾脏厚度。结果:治疗后,治疗组的HA、LN、PCⅢ、ⅣC明显改善(P<0.05),且改善程度优于对照组(P<0.05);治疗后,两组的LSM均明显下降(P<0.05),且治疗组的下降程度优于对照组(P<0.05);治疗后,治疗组的肝门静脉直径与脾脏厚度明显改善(P<0.05),和对照组比较,差异具有统计学意义(P<0.05)。结论:恩替卡韦联合复方鳖甲软肝片在肝脏纤维代谢和形态学改变方面具有抗纤维化的效用,提示其有助于改善慢性乙型肝炎肝硬化失代偿期患者的病情及预后。
Objective:To explore the effect of entecavir combined with compound Biejia Ruangan tablet for hepatic fibrosis indexes of patients with chronic hepatitis B cirrhosis at loss compensation period.Methods:63 patients with chronic hepatitis B cirrhosis at loss compensation period were randomly divided into the treatment group and the control group.The two groups were given conventional therapy in department of internal medicine,the treatment group added entecavir combined with compound Biejia Ruangan tablets,the control group added adefovir dipivoxil capsules,we compared the hyaluronic acid(HA),laminin(LN),type Ⅲ procollagen(PC Ⅲ),type Ⅳ collagen(Ⅳ C),liver stiffness(LSM),hepatic portal vein diameter and spleen thickness of the two groups before and after treatment.Results:After treatment,the HA,PCⅢ,LN and ⅣC of the treatment group were significantly improved(P<0.05),and the improvement degree was better than the control group(P<0.05).After treatment,the LSM of the two groups were significantly decreased(P<0.05),and the decrease of the treatment group was better than that of the control group(P<0.05).After treatment,the diameter of hepatic portal vein and spleen thickness were significantly improved in the treatment group(P<0.05),compared with the control group,the difference was significant(P<0.05).Conclusion:Entecavir combined with compound Biejia Ruangan tablet had the effect of anti fibrosis in the liver fiber metabolism and morphology changes,this suggested that it was helpful to improve the condition and prognosis of patients with chronic hepatitis B cirrhosis at loss compensation period.
