摘要
目的:探讨改良Hodge试验检测KPC型碳青霉烯酶最佳底物及其临床应用。方法:收集102株多重耐药的肠杆菌科细菌,选择使用亚胺培南、美罗培南、厄他培南为改良Hodge试验的不同底物,筛选产碳青霉烯酶表型株。结果:以厄他培南为最佳检测底物在102株肠杆菌科细菌中检出12株改良Hodge试验阳性株。8株产丝氨酸类(SCHBLS)碳青霉烯酶,4株产金属类(MCHBLS)碳青霉烯酶。12株改良Hodge试验阳性菌体外药敏试验结果表明,对临床常用抗菌药物均耐药。结论:改良Hodge试验的最佳底物为厄他培南。对耐碳青霉烯酶类抗生素的肠杆菌科细菌进行表型检测能更好地降低多重耐药菌的流行性,指导临床合理、有效地联合应用抗菌药物,提高临床疗效。
Objective:To explore the optimal substrate and the clinical application of modified Hodge test for the detection ofKPC type carbapenemases.Methods:102 strains of multi drug resistant strains of enterobacteriaceae bacterium were collected,imipenem,meropenem and ertapenem were used as substrate for modified Hodge test,to screen the phenotype of production ofcarbapenemases.Results:In 102 strains of enterobacteriaceae bacterium with ertapenem as the optimum detection substrate,themodified Hodge tests of 12 strains were positive.8 strains were carbapenemases for production of serine.4 strains werecarbapenemases for production of metal.The results of drug sensitivity test in vitro of 12 strains with positive modified Hodge testshowed that all of them were resistant to common antibiotics.Conclusion:The best substrate of modified Hodge test was ertapenem.Phenotypic testing for enterobacteriaceae bacterium with resistant for carbapenem antibiotics can reduce the prevalence of multidrug resistant bacteria,guide clinical rational and effective combination of antibacterial drugs and improve the clinical efficacy.
