摘要
目的系统评价他汀类药物治疗扩张型心肌病(DCM)的疗效。方法计算机检索Pub Med、The Cochrane Library、EMbase、CNKI、VIP、万方等数据库,收集他汀治疗DCM的随机对照试验文献,检索从建库至2015年10月的文献。结果共纳入26项研究包括1565例患者,Meta分析结果显示,与对照组相比,应用他汀治疗DCM能够显著提高左室射血分数(LVEF)(MD=5.62,95%CI:4.02~7.23,P<0.001,I2=89%)及运动耐量(六分钟步行试验,MD=36.78,95%CI:16.98~56.59,P<0.001,I2=94%),降低左室舒张末期内径(LVEDD)(MD=-3.32,95%CI:-5.14^-1.50,P=0.0004,I2=87%)、血浆脑钠肽(BNP)(MD=-66.52,95%CI:-106.80^-26.24,P=0.001,I2=99%)、C反应蛋白(CRP)(MD=-2.15,95%CI:-2.84^-1.46,P<0.001,I2=93%)及白介素-6(IL-6)(MD=-6.50,95%CI:-9.70^-3.29,P<0.001,I2=94%)。结论在常规药物治疗的基础上,加用他汀可能在一定程度上改善DCM患者的心功能,增加LVEF,降低炎症水平,改善心室重构。
Objective To review systematically the curative effect of statins in treatment of dilatedcardiomyopathy (DCM). Methods The databases of PubMed, Cochrane Library, EMbase, CNKI, VIP Database andWanFang Database were retrieved for collecting the literature of randomized controlled trials (RCT) on treatmentof DCM with statins from database establishment time to Oct. 2015. Results There were totally 26 RCT includedinvolved 1565 patients. The results of Meta-analysis showed that left ventricular ejection fraction (LVEF, MD=5.62,95%CI: 4.02~7.23, P<0.001, I2=89%) and 6-minute walk test (MD=36.78, 95%CI: 16.98~56.59, P<0.001,I2=94%) increased, and left ventricular end-diastolic inner diameter (MD=-3.32, 95%CI: -5.14~ -1.50, P=0.0004,I2=87%), brain natriuretic peptide (MD=-66.52, 95%CI: -106.80~ -26.24, P=0.001, I2=99%), C-reactive protein(MD=-2.15, 95%CI: -2.84~ -1.46, P<0.001, I2=93%) and interleukin-6 (MD=-6.50, 95%CI: -9.70~ -3.29,P<0.001, I2=94%) decreased in statins group compared with control group. Conclusion The administration ofstatins based on routine drugs can improve heart function and LVEF, reduce the level of inflammation and relieveventricular remodeling in patients with DCM.
