深低温低流量转流对N-甲基-D-天冬氨酸受体表达的影响

摘要 目的 观察乳猪深低温下不同程度低流量转流前后大脑皮层N 甲基 D 天冬氨酸 (NMDA)受体表达的变化 ,探讨深低温下不同流量转流后脑损害发生的可能机制。方法 取 3~ 4周龄乳猪 18头 ,随机均分为对照组、2 5ml·min-1·kg-1流量组和 5ml·min-1·kg-1流量组。分别于转流前 ,深低温低流量转流 1h后及复温至肛温 (36°C)再常温转流 2h后 (C点 ) ,经颅底钻孔取大脑皮质 ,用RT PCR法观察皮质NMDA受体表达的变化。结果 在 5ml·min-1·kg-1流量组C点时NMDA受体表达与对照组及该组起始值比较呈显著升高 ,而 2 5ml·min-1·kg-1流量组转流后与转流前该受体表达的差异无显著性。结论 深低温时过低流量转流后可引起大脑皮质NMDA受体表达升高 。 Objective To observe the changes of the N methyl D aspartate receptor expression in cerebral cortex of piglet models, undergone cardiopulmonary bypass surgery of hypothermic low flow status(DHLF)and to investigate the possible mechanism of the cerebral insult after DHLF. Methods Eighteen piglets were divided into three groups: the control group and the other two groups with a flow of 25 ml·min -1 ·kg -1 and 5 ml·min -1 ·kg -1 ,respectively .The cerebral cortices were taken out at the beginning(A),one hour after DHLF (B) and 2 hours after being rewarmed to 36°C (C) respectively. Results The expression of the N methyl D aspartate receptor in the group with the flow of 5 ml·min -1 ·kg -1 increased significantly at C as compared with the beginning value and that of the control group concomitently. No difference was shown in the group with the flow of 25 ml·min -1 ·kg -1 at B and C. Conclusion After the long duration perfusion of the deep hypothermic low flow status,the expression of the N methyl D aspartate receptor increases significantly, thereby initiating the secondary cerebral damage.
