摘要
目的研究无创血流动力学监测(ICG)指导儿童脓毒症患儿体液治疗的效果。方法选择2016年3月至2018年3月60例儿童脓毒症患儿为研究对象,随机将其分为观察组(ICG+常规治疗)与对照组(常规治疗),每组30例。观察两组治疗效果。结果复苏6 h后,两组血流动力学指标心率(HR)、平均动脉压(MAP)、中心静脉压(CVP),微循环指标血乳酸含量(LAC)、中心静脉血氧饱和度(ScvO2)、动静脉血二氧化碳分压(Pv-CO2)差异均未见统计学意义(P>0.05)。观察组尿量显著高于对照组(P<0.05)。观察组去甲肾上腺素及多巴胺丁胺用量明显多于对照组(P<0.05),ICU入住时间明显短于对照组(P<0.05)。结论 ICG监测能准确管理儿童脓毒症患儿体液治疗,有效缩短其ICU治疗时间,为患儿疾病治疗与评估提供有效参考。
Objective To study the effects of non-invasive hemodynamic monitoring(non-invasive impedance cardiography,ICG)on the guidance of fluid therapy in children with sepsis.Methods Sixty children with sepsis from March 2016 to March 2018 were included in the study and were randomly divided into observation group(ICG+conventional treatment)and control group(conventional treatment),with 30 cases in each group.The treatment effects of the two groups were observed.Results After 6 hours of resuscitation,there were no significant differences in heart rate(HR),mean arterial pressure(MAP),central venous pressure(CVP),blood lactate content(LAC),central venous oxygen saturation(ScvO2),arteriovenous partial pressure of carbon dioxide(Pv-CO2)between the two groups(P>0.05).The urine volume in the observation group was significantly higher than that in the control group(P<0.05).The dosage of norepinephrine and dopamine in the observation group was significantly higher than that in the control group(P<0.05),and the ICU stay time was significantly shorter than that in the control group(P<0.05).Conclusions ICG monitoring can accurately manage the fluid therapy of children patients with sepsis,effectively shorten the ICU treatment time,and provide effective reference for the treatment and evaluation of children’s diseases.
