可摘局部义齿卡环对基牙牙面致龋菌的影响 被引量:6

Quantitative Changes of Cariogenic Bacteria on Abutment Teeth Surfaces Related to Clasps of Removable Partial Dentures

摘要 目的 了解卡环戴入前后基牙牙面微生态平衡的变化 ,并探讨不同卡环引起上述变化的差异。方法 检测备牙前、戴牙一个月及三个月时铸造卡环和锻丝卡环所在基牙牙面的可培养菌总量以及变形链球菌、粘性放线菌和乳杆菌组成比的变化。结果 与备牙前相比 ,戴牙一个月及三个月时 ,两种卡环所对应的基牙牙面的检出菌总量以及变形链球菌和粘性放线菌的组成比均明显增高 ,其差异具有统计学意义 (P<0 .0 5 ) ,且锻丝卡环所对应的牙面比铸造卡环所对应的牙面的变形链球菌组成比增高更明显 (P<0 .0 5 )。结论 卡环的戴入在牙面形成新的滞留区 ,促进细菌沉积及致龋菌繁殖。与铸造卡环相比 。 Objective This study was designed to find out the changes in microecology on abutment teeth surfaces after the wearing of clasps, and in turn to explore the difference of such changes caused by wrought wire clasps and casting clasps. Methods Sixteen patients who were ready to accept removable partial dentures were selected and divided into 2 groups. One group used wrought wire clasps while the other group used casting clasps. Then at the following three points: before dentures were inserted, one month after wearing and three months after wearing, the total cultivable flora were counted and the percentages of the three major cariogenic bacteria on the abutment teeth surfaces were calculated. Results No matter what type of clasps was selected, the wearing of clasps enhanced the accumulation of microflora on abutment teeth surfaces. The percentages of Streptococcus mutans (S.mutans) and Actinomyces viscosus (A.viscosus) detected on abutment teeth surfaces also increased significantly ( P <0.05). When such changes were compared, we found that the increment of S.mutans caused by wrought wire clasps was higher than that caused by casting clasps ( P <0.05). Conclusion The wearing of clasps forms new retention areas on abutment teeth surfaces, and in turn enhances the deposit of microbe and facilitates the growth of cariogenic bacteria. Compared with such changes caused by casting clasps, the changes related to wrought wire clasps are more evident.
