摘要
目的基于2015年比尔斯标准和电话随访分析门诊老年患者地高辛用药剂量,为门诊老年患者(≥65岁)合理开具地高辛处方剂量提供参考。方法回顾性分析北京大学首钢医院2017年3月至2018年5月门诊患者含有地高辛的所有处方,共83例老年患者的临床资料,根据美国老年医学会2015年比尔斯标准中地高辛的推荐剂量,参照对应的处方剂量,通过电话随访的方式了解实际用药剂量情况,并结合患者年龄评价地高辛的处方剂量。结果 83例患者地高辛处方剂量中,0.5 mg/d者4.82%,0.25 mg/d者86.75%,≤0.125 mg/d者8.43%;进行电话随访后发现,实际用药情况中,0.5 mg/d者0%,0.25 mg/d者18.07%,≤0.125 mg/d者81.93%。结论门诊服用地高辛老年患者,实际用药剂量低于医师处方剂量的情况较为多见,大部分患者用量在2015年比尔斯标准内,因此要做好处方剂量合理性的管控,同时做好患者合理用药的监督指导。
Objective To analyze the dose of digoxin in elderly outpatients based on the Biers standard of 2015 and telephone follow-up,in order to provide a reference for the prescription dose of elderly patients(≥65 years old).Methods A retrospective analysis of the clinical data of 83 elderly patients with all prescriptions of digoxin in outpatients from March 2017 to May 2018 of Peking University Shougang Hospital was conducted according to the American Geriatrics Association’s 2015 Bills standard.According to the corresponding prescription dose,the actual dosage was obtained by telephone follow-up,and the prescription dose of digoxin was evaluated according to the age of the patient.Results Among the 83 patients,the prescription dose of digoxin was 4.82%for 0.5 mg/d,86.75%for 0.25 mg/d,and 8.43%for≤0.125 mg/d.However,after telephone followup,it was found that 0.5 of the actual medication was used.0%for mg/d,18.07%for 0.25 mg/d,and 81.93%for≤0.125 mg/d.Conclusion In the elderly patient who take digoxin in the outpatient clinic,the actual dose is lower than the prescribed dose of the doctor.Most of the patients are within the 2015 Bills standard.Therefore,it is necessary to control the rationality of the prescription dose and at the same time improve patient supervision and guidance of rational drug use.
