摘要
目的探讨腹腔镜手术在阑尾周围脓肿临床治疗中应用效果,观察Ⅰ期手术疗效。方法选取2017年4月至2018年6月本院收治的124例阑尾周围脓肿患者为研究对象,随机分为观察组(腹腔镜手术)和对照组(开腹手术),各62例。观察两组患者的治疗效果及安全性,包括切口长度、术中出血量、引流时间、肛门排气时间、下床活动时间、住院时间,以及并发症发生率。结果观察组患者切口长度、术中出血量、引流时间分别为[(2.24±0.38)cm、(19.25±4.02)mL、(2.89±0.42)d],显著低于对照组[(5.81±0.61)cm、(44.18±6.54)mL、(5.09±0.38)d,P <0.05];观察组肛门排气时间、下床活动时间及住院时间分别为[(24.14±2.05)h、(22.08±1.84)h、(3.87±0.26)d],显著短于对照组[(29.16±2.35)h、(36.56±3.42)h、(7.05±0.34)d,P <0.05];观察组并发症发生率为3.23%,显著低于对照组(16.13%,P <0.05)。结论对阑尾周围脓肿患者采用腹腔镜Ⅰ期手术治疗,可有效改善患者病情,降低术后并发症及复发风险。
Objective To investigate the effect of laparoscopic surgery in the treatment of periappendiceal abscess and to observe the effect of primary operation.Methods From April 2017 to June 2018,124 patients with periappendiceal abscess were divided into observation group(laparoscopic surgery)and control group(open surgery),62 cases in each group.The therapeutic effect and safety of the two groups were observed,including the length of incision,the amount of bleeding,the time of drainage,the time of exhausting anus,the time of moving out of bed,the time of hospitalization,and the incidence of complications.Results The incision length,intraoperative blood loss and drainage time in the observation group[(2.24±0.38)cm,(19.25±4.02)mL,(2.89±0.42)d]were lower than those in the control group[(5.81±0.61)cm,(44.18±6.54)mL,(5.09±0.38)d,P<0.05].The anal exhaust time,ambulation time and hospitalization time in observation group[(24.14±2.05)h、(22.08±1.84)h,(3.87±0.26)d]were shorter than those in control group((29.16±2.35)h,(36.56±3.42)h,(7.05±0.34)d,P<0.05)The incidence of complications in the observation group(3.23%)was significantly lower than that in the control group(16.13%,P<0.05).Conclusion Laparoscopic surgery at the primary stage for periappendiceal abscess can effectively improve the patient’s condition and reduce the risk of postoperative complications and recurrence.
