摘要
目的:探讨血清瘦素浓度与中老年人高血压及相关因素的关系。 方法:测定 36例中老年高血压男性患者 (高血压组 )和 32例健康对照者 (对照组 )的空腹血糖 (FBG)、血清总胆固醇 (Tch)、甘油三酯 (TG)、高密度脂蛋白胆固醇 (HDL - C)、低密度脂蛋白胆固醇 (L DL - C)、空腹胰岛素 (FINS)及血清瘦素浓度值。空腹胰岛素 (FINS)及血清瘦素用放射免疫法测定。同时按全国高血压普查方法测量收缩压、舒张压、身高、体重、心率 ,计算胰岛素敏感指数 (ISI)、体重指数 (BMI)。结果 :(1)高血压组与对照组比较 ,年龄、空腹胰岛素、空腹血糖、心率、胰岛素敏感指数、体重指数、血清总胆固醇、甘油三酯、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇间有可比性 ,均无统计学差异 ,其血清瘦素浓度也无统计学差异 (P >0 .0 5 )。 (2 )血清瘦素浓度与 FINS呈正相关 (r =0 .4 4 1,P <0 .0 1) ,与ISI呈负相关 (r =- 0 .2 99,P <0 .0 5 ) ,与血压无明显相关 (P >0 .0 5 )。 (3) FINS、FBG是影响中老年男性血清瘦素浓度的显著因素。 结论 :血清瘦素浓度与血压无直接相关关系 。
Objective: To explore the relationship between serum leptin, hypertension and relevant factors in elderly persons. Methods: Serum leptin concentrations and serum insulin level were measured by radioimmunoassay. The level of fasting blood glucose(FBG), fasting insulin(FINS), triglycerides(TG), total cholesterol (T ch), low density lipoprotein cholesterol (LDL ch), high density lipoprotein cholesterol (HDL ch) were determined in 36 elderly persons with essential hypertension(EHT) and 32 elderly persons with normotensives (NT). Heart rate, weight, height, systolic blood pressure and diastolic blood pressure were measured. Body mass index (BMI), and insulin sensitivity index(ISI) were calculated. Results: (1) FBG, FINS, TG, T ch, LDL ch, HDL ch, BMI, and ISI in elderly males were not significantly different between the EHT and NT groups and serum leptin concentrations in elderly males were also not significantly different between the EHT and NT groups (P>0.05). (2)The relationship between these parameters in elderly males was studied by univariate correlation analysis. INS (r=0.441, P<0.01) were positively correlated with the serum leptin level and ISI (r=-0.2999, P<0.05) were negative correlated with it. (3) The multiple regression analysis indicated that FINS and FBG were closely correlated with leptin in elderly males. Conclusion: Serum leptin concentrations in elderly males are not directly related to blood pressure and are correlated with insulin resistance.
