摘要
目的 :评价一期膀胱粘膜或包皮内板在尿道下裂修复术中的应用和并发症的预防。方法 :尿道下裂患者 16 4例 ,年龄 1.5～ 2 5岁 ,初次手术病例组 138例 ,采用包皮内板尿道成形术 47例 ,膀胱粘膜尿道成形术 91例 ;再次手术病例组 2 6例 ,全部采用膀胱粘膜尿道成形术。结果 :术后随访 6个月～ 7年。所有病例一次手术成功率 (94.5 % )。尿瘘 9例都位于冠状沟下 ,初次手术组与再次手术组的成功率 (94.9%∶92 .3% )差异无显著性意义 ,使用膀胱粘膜与包皮内板尿道成形术成功率 (94.1%∶95 .7% )差异无显著性意义。结论 :无论是那种类型的尿道下裂 ,正确应用包皮内板或膀胱粘膜尿道成形术能取得良好的疗效 ,手术成功的关键不在于手术的方法 。
Purpose:To evaluate free internal preputial flap and bladder mucosa graft urethroplasty for varied kinds of hypospadias and prevention of post-operative complication.Method: From Oct. 1985 to Dec. 2001, 164 cases of hypospadias were treated by free internal preputial flap and bladder mucosa urethroplasty. The age of the patients ranged from 18 months to 25 years with a mean of 3. 8 years. In 138 cases first repair group, free internal preputial flap urethroplasty was undertaken in 47 cases, and free bladder mucosa graft urethroplasty in 91 cases . The 26 cases secondary repair group was conducted by senior doctors using free bladder mucosa graft urethroplasty. All cases were followed up 6 months to 7 years.Result: The cosmetic results and voiding function were satisfactory. Urethral fistula occurred in 9 cases( 5.5%), urethral orifice cicatricial stricture in 5 ( 3.0%). The success rates between first ( 94.9%) and secondary group( 92.3%)show no significant differences, neither between bladder mucosa graft urethroplasty( 94.1%)and free internal preputial flap( 95.7%).Conclusion: Bladder mucosa graft urethroplasty and free preputial graft can be used in varied kinds of hypospadias. The key to success depends on operators skills and preciseness to handle the procedure.
