中心性浆液性脉络膜视网膜病变患者远近立体视功能 被引量:4

A study in the comparison of far and near stereopsis function in central serous chorioretinopathy cases

摘要 目的 :比较“中浆”患者远近立体视功能损害的临床特点。方法 :对 48例 (5 1眼 )中浆患者按常规方法检查视力、屈光、Amsler方格卡、双眼影像、眼底FFA或OCT ;用同视机检查患者双眼同时知觉、融合范围及定性立体视 ;采用同视机随机点画片及《立体视觉检查图》检测患者的远近立体视功能及抑制性暗点。结果 :48例患者除中心视力下降、视物变形和有中心暗点等单眼视功能受损外 ,还有双眼影像不等及双眼视功能不同程度的损害。双眼视功能受损中 ,双眼同时视障碍为 2 .1% ,双眼融合视缺失为 4.3% ,而双眼定性立体视丢失为 16 .7%。采用随机点画片定量检测发现 ,远距离交叉视差锐度和非交叉视差锐度及近距离立体视锐度达到正常者分别为 2 8例 (占 5 8.3% )、2 4例 (占 5 0 .0 % )和 14例(占 2 9.2 % )。显示近立体视损害较远立体视更明显 ,差异有显著性 (P <0 .0 5 )。病愈后三项功能恢复中心立体视者分别为 47例 (占 97.9% )、46例 (占 95 .8% )和 45例 (占 93.8% )。病愈前后远近距离三项立体视锐度达到正常者的比例以及平均视锐度值 ,差异有显著性 (P <0 .0 1)。影响立体视功能的主要原因是两眼视力相差大 ,双眼同时视像差大及有抑制性中心暗点。结论 :中浆患者视功能受损表现是多方面的。 Objective:To study and compare the clinical characteristics of impaired far and near stereopsis function in patients with CSC.Methods:Refractions and amsler grid tests were performed on 48 cases. Binocular images(FFA or OCT),binocular fusion sense,fusion range,and the nature of stereopsis were examined with a synoptophore. Far and near stereoscopic function and inhibitory scotomas of patients were examined by using random dot stereograms in the synoptophore and stereoscopic charts.Results:In the 48 cases,central vision decreased due to metamorphopsia and scotoma in the affected eye. Both binocular aniseikonia and binocular visual function showed different degrees of damage. For binocular visual function,2.1% had a disturbance in binocular simultaneous perception,4.3% had defective binocular fusion,and 16.7% had a qualitative stereopsis loss. Patients were examined quantitatively using random dot stereograms in the synoptophore,results showed that far crossed stereo acuity,non-crossed stereo acuity and near stereo acuity were normal in 28 cases(58.3%),24 cases(50.0%) and 14 cases(29.2%),respectively. These results show that impairment in near stereoscopic vision was greater than that for far stereoscopic vision. The difference was significant(χ 2=8.73,P=0.013,P<0.05). After recovery,central stereo acuity for the 3 functions was restored in 47 cases(97.9%),46 cases(95.8%) and 45 cases(93.8%),respectively. After recovery of far and near vision,the three stereo acuity functions reached normal levels,both the levels mentioned above and the average value for visual acuity showed statistically significant improvement(P<0.01). The main factors that affect stereoscopic function are a large difference in vision between the two eyes,a large difference in the image for binocular simultaneous perception and central scotoma.Conclusion:Damage to visual function in CSC can be expressed in many ways.
