摘要
目的 比较经皮肾镜与腹腔镜下行肾囊肿去顶术治疗单纯性肾囊肿的疗效及安全性.方法 回顾性分析2012年4月至2016年12月在本科行经皮肾镜下行肾囊肿去顶减压术（经皮肾镜组）和后腹腔镜下行肾囊肿去顶减压术（腹腔镜组）的40例患者临床资料,对比两组患者手术时间、术中出血量、术后并发症、住院时间、随访6个月后囊肿复发及患者满意度等指标.结果 40例患者手术均获成功,两组术中出血量、住院时间、手术时间、术后并发症发生率差异无统计学意义.经皮肾镜组术后引流量较腹腔镜组多,两组比较有统计学意义（P＜0.001）.随访6个月,两组患者均无复发,但经皮肾镜组患者满意度优于腹腔镜组（P＜0.001）.结论 对于治疗单纯性肾囊肿,经皮肾镜与腹腔镜下行肾囊肿去顶减压术均安全有效,随着经皮肾镜手术经验的增加,经皮肾镜下行肾囊肿去顶术也是一种可供选择的手术方式.
Objective To compare the efficacy and safety of percutaneous nephroscopy surgery and laparoscopic surgery for the treatment of simple renal cysts.Methods Forty patients with simple renal cysts were retrospectively analyzed from April 2012 to December 2016.They were treated by laparoscopic renal cyst unroofing decompression （laparoscopic group,20 cases） and percutaneous nephroscopy cyst unroofing decompression （percutaneous nephroscopy group,20 cases）.The mean operation time,operation bleeding,the rate of postoperative complications （infection,fever,urine leakage）,hospitalization time after operation,and follow-up data after 6 months were compared between the two groups.Results All the 40 operations were successfully performed.The clinical cure rate of two groups was 100％,the mean operation time,the operation bleeding,postoperative complications and hospitalization time after operation between the two groups were not significant difference.The postoperative drainage of percutaneous nephroscopy group was more than that of laparoscopic group,the difference was statistically significant （P ＜0.001）.During the follow-up of 6 months,no recurrence was found in the two groups,patient＇s satisfaction of percutaneous nephroscopy group was better than laparoscopic group,the difference was statistically significant （P ＜ 0.001）.Conclusions Percutaneous nephroscopy and laparoscopic surgery are effective and security for the treatment of simple renal cyst.At present,percutaneous nephroscopy cyst unroofing decompression is an alternative method and is worth promoting with the increase of experience.
