青少年第五跖骨近端骨骺与骨折在数字X线摄影特点及临床意义

摘要 目的观察分析青少年第五跖骨近端骨骺、骨折的数字X线摄影图像特点,为临床鉴别诊断提供参考。方法方便收集2015年1月—2018年3月于该院骨科就诊并被诊断为第五跖骨近端骨骺或骨折的7~14岁青少年150例,根据诊断结果分为骨骺组(n=108)、骨折组(n=42),分析两组患者的数字X线摄影图片在形态学及骨折类型上的特点。结果 (1)在骨骺组患者的数字X线影像中,鱼鳞片状为最主要的形态学特点,占74.1%,显著高于粟粒状16.7%(χ~2=69.503,P=0.000)、卵圆形2.8%(χ~2=113.019,P=0.000)和不规则形状6.5%(χ~2=99.772,P=0.000);(2)骨折组的数字X线摄影结果显示,3型横行骨折占比最高,达61.9%,其明显高于1型粗隆尖部骨折28.6%(χ~2=8.121,P=0.004)和2型斜型骨折11.9%(χ~2=20.450,P=0.000)。结论青少年第五跖骨近端骨骺的数字X线图像多表现为骺线及骨干平行,多呈鱼鳞片状;骨折表现为骨块形态不规则,折线锐利,以横行为主,在临床诊断中应参考数字X线摄影结果对二者进行鉴别,以提高临床诊断准确率。 Objective To observe and analyze the features and clinical significance of proximal epiphysis of the fifth metatarsal in adolescents and fracture in the digital radiography thus providing reference for the clinical differential diagnosis. Methods 150 cases of adolescents with proximal epiphysis of the fifth metatarsal in our hospital from January 2015 to March 2018 were convenient selected and divided into two groups according to the diagnosis results, including the epiphysis group with 108 cases and fracture group with 42 cases, and the features of digital radiography in the morphology and fracture types of the two groups were analyzed. Results 1.The scaly flake was the most prominent morphologic feature in the digital radiography, accounting for 74.1%, which was obviously higher than the military shape 16.7%(χ^2=69.503, P=0.000) , orbicular-ovate shape 2.8%(χ^2=113.019, P=0.000) and irregular shape 6.5%(χ^2=99.772, P=0.000); 2.And the digital radiography results in the fracture group showed that the ratio of transverse fracture of type 3 was the highest, accounting for 61.9%, which was obviously higher than that of type 1 tip fracture of trochanter 28.6%(χ^2=8.121, P=0.004) and type 2 oblique fracture11.9%(χ^2=20.450, P=0.000). Conclusion The digital radiography of proximal epiphysis of the fifth metatarsal in adolescents is mostly manifested as epiphysis line and backbone parallel, mostly in scaly flake shape, and the fracture is manifested as the irregular shape of bone block, mostly in parallel shape, and we should distinguish the two according to the digital radiography in clinical diagnosis to improve the clinical diagnosis accurate rate.
