摘要
目的针对早中期的强直性脊柱炎(AS)患者,分别采取CT引导下针刀松解术、射频热凝术、关节腔局部注射治疗方案,通过检测指标并结合MRI的影像学改变客观评估针刀治疗AS的疗效。方法将90例患者随机分成针刀松解术组(A组)30例,射频热凝术组(B组)30例,局部注射组(C组)30例。3组患者均配合常规药物,进行为期半年的治疗,通过指标(VAS、BASDAI、BASFI、扩胸度、Schober、血沉、C反应蛋白)观察评估疗效和骶髂关节MRI(SPARCC)等情况。结果 3组治疗6月后VAS、 BASDAI、BASFI、SPARCC评分语A组与其他两组相比,A组得分明显优于B、C组,结果差异有统计学意义(P<0.05)。结论针刀松解术具有操作便捷、效果显著等优点,具有临床推广和应用价值。
Objective to assess the curative effect through indicators and the MRI scores by CT guided acupotomy release,radiofrequency therapy （RFT） and local injection of the joint cavity for the early or middle patients of ankylosing spondylitis（AS）. Methods 90 cases of ankylosing spondylitis patients were collected and divided into the needle-knife group （A group）,the RFT group （B group） and the injection group （C group）. All the patients in the 3 groups were treated with conventionaldrugs for half a year, and the results were evaluated by the indicators including VAS, BASDAI, BASFI, chest expansion degree,Schober, erythrocyte sedimentation rate and C-reaction protein and the MRI of the sacroiliac joint. Results VAS, BASDAI,BASFI and SPAR Results CC scores in group A were significantly better than those in group B and C, which had statisticallysignificant differences after 6 months of treatment. Conclusions The method of acupotomy with convenient, effective featuresfor AS patients had the value for the further clinical promotion and application.
