摘要
目的探讨超敏C-反应蛋白(hs-CRP)和中性粒细胞/淋巴细胞比值(NLR)与类风湿性关节炎的相关性。方法选取类风湿性关节炎(RA)患者210例,其中活动组102例和非活动组108例,同时选取110例健康者为对照组,收集空腹血样本用于血常规、hs-CRP、免疫球蛋白、补体C3(C3)、补体C4(C4)、类风湿因子(RF)、血沉(ESR)、抗CCP抗体等检测,根据血常规结果计算中性粒细胞/淋巴细胞比值。结果 RA非活动组、RA活动组NC、NLR、hs-CRP、ESR、IgG、C3、C4、抗CCP抗体和RF均高于对照组,而LC均低于对照组。Logistic回归分析显示,hs-CRP和NLR均是RA病情活动的危险因素(P <0. 01)。hs-CRP的AUC为0. 776,截断值取3. 79 mg/L时,灵敏度为78. 40%,特异度为66. 70%; NLR的AUC为0. 809,截断值取3. 04时,灵敏度为80. 70%,特异度为68. 80%。结论 hs-CRP和NLR均是RA病情活动的独立危险因素,其中NLR对RA病情活动的诊断预测价值最大。在临床上,对于NLR> 3. 04的RA患者,应早期治疗,有可能延缓病情的进展。
Objective To explore the relationship between high sensitive C-reactive prot( hs-CRP),neutrophil-to lymphocyte ratio and rheumatoid arthritis. Methods 210 patients with rheumatoid arthritis( RA) were divided into patients with RA activity( n = 102) and RA non activity( n = 108). 110 healthy people were also enrolled in our study. Fast blood samples were collected for the detections of blood routine,hs-CRP,immunoglobulin,complement C3( C3),complement C4( C4),rheumatoid factor( RF),erythrocyte sedimentation rate( ESR),anti-CCP antibody. And neutrophil/lymphocyte ratio( NLR) was calculated according to blood routine results. Results Compared with the control group,the levels of NLR,hs-CRP,ESR,IgG,C3,C4,anti-CCP,antibodies and RF were significantly higher while LC was lower in the RA activity group and the RA non activity group. Logistic regression analysis showed that hs-CRP and NLR were risk factors of the disease activity of RA( P 0. 01). When AUC of hs-CRP was 0. 776 and the cut off value was set at 3. 79 mg/L,sensitivity was 78. 40% and specificity was 66. 70%. When AUC of NLR was 0. 809 and the cut off value was set at 3. 04,sensitivity was 80. 70% and specificity was 68. 80%. Conclusion hs-CRP and NLR are independent risk factors for RA disease activity. NLR has the greatest diagnostic value to RA activity,patients whose NLR is over 3. 04 need therapy earlier in order to prevent the progress of RA.
