慢性创面感染患者细菌学调查及耐药性分析 被引量:5

摘要 目的研究慢性创面感染患者创面的细菌分布特点及细菌耐药情况,为临床用药提供参考。方法回顾性分析我院烧伤整形科2015年1月~2017年12月收治的慢性创面感染患者139例,对创面分泌物的细菌培养结果及药敏试验结果进行分析。结果 139例慢性创面患者的创面分泌物共培养出细菌菌株251株,其中革兰阳性菌134株,构成比为53.39%,金黄色葡萄球菌检出率最高,为30.28%。革兰阴性菌115株,构成比45.82%,大肠埃希菌检出率最高,为9.56%。真菌菌株2例,构成比为0.80%,均为近平滑假丝酵母。检出率较高的细菌菌种除对部分抗生素敏感外,对大多数抗生素均耐药。结论慢性创面感染常见细菌存在普遍耐药性,对慢性创面的治疗,应在充分了解细菌菌种及其耐药性的基础上合理选用抗生素,并进行适当的外科治疗。 Objective To study the distribution characteristics and resistance of bacteria in patients with chronic wound infection, provide reference for clinical treatment. Methods Retrospectively analysed 139 patients with chronic wound infections admitted to burn and plastic surgery department from Jan 2015 to Dec 2017. The results of bacterial culture of wound secretion and the results of drug sensitivity test were analyzed. Results A total of 251 bacterial strains were cultured from 139 chronic wounds, 134 were gram-positive bacteria, the constituent ratio was 53.39%, Staphylococcus had the highest detection rate(30.28%). Among which 115 were gram-negative bacteria, the composition ratio was 45.82%, the detection rate of Escherichia coli was the highes(t 9.56%). Two cases had fungal strain, the constituent ratio was 0.80%, they were both candida parapsilosis. Higher detection rate of bacterial species, in addition to some antibiotics sensitive, were resistant to most antibiotics. Conclusion The common bacteria in chronic wound infections are generally resistant. For the treatment of chronic wounds, rational use of antibiotics should be based on a thorough understanding of bacterial strains and their drug resistance, and appropriate surgical treatment should be adopted.
