摘要
目的 探讨血清脂蛋白a[LP（a）]、高密度脂蛋白（HDL）、糖类抗原72-4（CA72-4）水平与胃癌患者TNM分期的关联性及临床意义.方法 选取运城市中心医院2015年10月至2017年11月胃癌患者86例设为恶性组,86例胃良性疾病患者设为良性组,另选取同期于医院进行健康体检者86例设为对照组.入院后第2天抽取各组空腹静脉血4 ml,测定血清LP（a）、HDL、CA72-4水平,并给予恶性组患者对应治疗措施.统计恶性组、良性组、对照组及TNM分期胃癌患者和胃癌患者治疗前后血清HDL、LP（a）、CA72-4水平,并分析血清HDL、LP（a）、CA72-4水平与TNM分期相关性.结果 恶性组、良性组、对照组血清HDL、LP（a）、CA72-4水平比较差异未见统计学意义（P〈0.05）,且良性组血清LP（a）、CA72-4水平高于对照组（P〈0.05）,恶性组血清LP（a）、CA72-4水平高于良性组,HDL水平低于良性组（P〈0.05）;不同TNM分期胃癌患者间血清HDL、LP（a）、CA72-4水平比较差异未见统计学意义（P〉0.05）,且随着TNM分期增加,血清LP（a）、CA72-4水平不断增高,HDL水平不断降低;治疗后血清HDL水平高于治疗前,LP（a）、CA72-4水平低于治疗前（P〈0.05）;血清HDL水平与胃癌TNM分期存在明显负相关关系,血清LP（a）、CA72-4水平与胃癌TNM分期存在明显正相关关系（P〈0.05）.结论 胃癌患者血清HDL、LP（a）、CA72-4水平发生异常,且随着TNM分期增加,血清LP（a）、CA72-4水平不断增高,HDL水平不断降低,临床可通过检测上述指标水平对胃癌予以诊断及TNM分期评估,且可用于评估患者治疗效果,为进一步治疗方案的制定及调整提供参考依据.
Objective To investigate the relationship between the levels of serum lipoprotein a [LP （a）], high density lipoprotein （HDL）, carbohydrate antigen 72-4 （CA72-4） and TNM staging in patients with gastric cancer and the clinical significance .Methods From October 2015 to November 2017 , 86 cases of gastric cancer in Yuncheng Central Hospital were enrolled in the malignant group and 86 patients with benign gastric disease were selected as benign group .At the same time , 86 cases of healthy physical examination in Yuncheng Central Hospital were selected as control group .On the second day after admission , 4 ml of fasting venous blood were extracted from each group , and the levels of serum LP （ a） , HDL and CA72-4 were measured .The corresponding treatment measures were taken for patients in malignant group.The levels of serum HDL, LP （a） and CA72-4 before and after treatment of gastric cancer patients in the malignant group , benign group and different TNM stagings were recorded .The relationship between serum HDL , LP （ a） , CA72-4 and TNM staging was analyzed .Results There were no significant differences in the levels of serum HDL , LP （ a） or CA72-4 among the malignant group , the benign group and the control group （P〈0.05）.The levels of serum LP（a） and CA72-4 in benign group were higher than those in control group （ P〈0.05 ） .The levels of serum LP （ a ） and CA72-4 in the malignant group were higher than those in the benign group , and the HDL level was lower than that in the＆nbsp;benign group （ P〈0.05 ） .There were no significant differences in the leves of serum HDL , LP （ a ） , CA72-4 between gastric cancer patients with different TNM stages （ P〉0.05） , and with the increase of TNM stage, the levels of serum LP （ a ） and CA72-4 increased continuously , and the level of HDL decreased continuously .The serum HDL level after treatment was higher than that before treatment .The levels of LP （ a ） and CA72-4 were lower than those before tr
