摘要
目的探讨肝外胆管癌(extrahepatic cholangiocarcinoma,ECC)的磁共振成像(magnetic resonance imaging,MRI)、磁共振胰胆管成像(magnetic resonance cholagniopancreatography,MRCP)表现。方法回顾性分析27例ECC患者临床、影像学、实验室检查及病理资料。患者均行上腹部MRI平扫、增强扫描及MRCP。分析比较ECC的MRCP、磁共振扩散加权成像(diffusion weighted,DWI)影像表现,并运用DWI、MRCP前后对ECC的诊断符合率、敏感性、特异性、阳性预测值、阴性预测值进行比较。MRCP可精确定位3种ECC,结合DWI后提高ECC的定性诊断。结果常规MRI+MRCP+DWI的诊断符合率为88. 9%,高于常规MRI+MRCP的62. 9%,差异有统计学意义(P <0. 05);MRI平扫、增强扫描联合MRCP更有利于早期发现病灶,并且病灶的延迟强化及继发的胆管扩张更有利于ECC的诊断。结论上腹部MRI平扫、增强扫描联合MRCP,可了解有无肝脏转移、淋巴结是否受累等,对临床医师及早制订治疗计划有重要的指导意义。
Objective To investigate MRI（magnetic resonance imaging）manifestations of extrahepatic cholangiocarcinoma（ECC）.
Methods MRI findings,clinical symptoms,laboratory examination and the pathological data of 27 patients were retrospectively analyzed.All the cases underwent plain MR scanning, contrast-enhanced and MRCP.The imaging features of MRCP and DWI were analyzed and compared;diagnostic compliance rate,susceptibility,specificity,positive prediction,negative prediction with or without DWI were compared.MRCP can make an accurate positioning for ECC.
Results Diagnostic compliance rate of conventional MRI,MRCP and DWI is 88.9%, diagnostic compliance rate of conventional MRI and MRCP was 62.9%. The differences were statistically significant（ P 〈0.05）.Conclusion Plain MR scanning,contrast-enhanced and MRCP can contribute to comprehending whether cancer cells metastasize to the liver or lymph nodes. It has significant value for the accurate MRI diagnosis of extrahepatic cholangiocarcinoma.It is very helpful in the selection of treatment program.
