摘要
目的探讨股骨近端抗旋髓内钉(PFNA)治疗股骨粗隆间合并股骨颈基底部骨折的临床疗效。方法本研究回顾分析本院外科2011年1月至2017年12月之间收治72例股骨粗隆间合并股骨颈基底部骨折患者的临床资料,随机分为对照组和实验组,对照组实施动力髋螺钉(DHS)治疗,实验组实施股骨近端抗旋髓内钉(PFNA)治疗,对比两组患者临床疗效。结果实验组术后1个月、术后3个月和术后6个月前髋关节Harris评分结果均明显高于对照组(P<0.05)。实验组手术时间、骨折愈合时间和手术出血量少于对照组,且切口长度明显短于对照组(P<0.05)。实验组并发症发生率为8.33%,对照组并发症发生率为11.11%,两组比较差异存在统计学意义(P<0.05)。结论股骨粗隆间合并股骨颈基底部骨折患者接受股骨近端抗旋髓内钉治疗,手术效果较理想,且术后恢复良好,并发症较少。
Objective To investigate the clinical effect of proximal femoral nail (PFNA) in the treatment of intertrochanteric fracture with femoral neck basal fracture. Methods The clinical data of 72 patients with intertrochanteric femoral fractures associated with femoral neck basal fractures treated in our hospital from January 2011 to December 2017 were retrospectively analyzed. They were randomly divided into control group and experimental group. The control group was treated with DHS and the experimental group was treated with PFNA. Treatment, compared two groups of patients with clinical efficacy. Results The Harris score of the experimental group was significantly higher than that of the control group at 1 month, 3 months and 6 months after operation (P〈0.05). The operation time, fracture healing time and bleeding volume of the experimental group were less than those of the control group, and the incision length was significantly shorter than that of the control group (P〈0.05). The incidence of complications was 8.33% in the experimental group and 11.11% in the control group. There was significant difference between the two groups (P〈0.05). Conclusion The proximal femoral anti-rotation intramedullary nail is effective in the treatment of intertrochanteric femoral fracture combined with femoral neck basal fracture, with good postoperative recovery and fewer complications.
