摘要
目的探讨高脂血标本对临床生化、血凝等指标的干扰及应对策略.方法收集300份正常血清与血浆样本,加入脂肪乳后使之成为脂肪血样本,比较脂肪乳加入前后血清生化指标ALT、AST、TP、CK-MB、Cr、UA、AMY、BUN、ALB、LDH、GGT及血凝指标PT、APTT、D-二聚体的差异.采用有机溶剂乙醚及正己烷对脂肪血标本进行处理,分析几种处理方法对脂肪血生化、血凝指标检测结果的改善情况.结果加入脂肪乳后血清样本与原血液样本相比,生化指标有明显改变,差异具有统计学意义(P<0.05).经过有机溶剂乙醚处理后,ALT、AST、TP、CK-MB、Cr有明显改善.加入脂肪乳后血浆样本的血凝指标PT、APTT无法检测,经正己烷处理后可正常进行检测.结论采取有效措施提高高脂血标本质量,消除脂肪给临床检验生化及血凝指标带来干扰以避免发出错误检验报告,造成临床医师误诊具有重要的临床意义.
Objective To observe the interference and countermeasures of hyperlipidemic specimens on the indexes of clinical biochemistry, hemagglutination, etc. Methods 300 copies of normal serum and plasma samples were selected and were made fat blood samples by adding fat emulsion and the differences of serum biochemical indexes of ALT, AST, TP, CK-MB, Cr, UA, AMY, BUN, ALB, LDH, GGT and hemagglutination were compared before and after the addition of fat milk. The organic solvent ethyl ether and n-hexane were used to treat the fat blood samples,and the improvement of the results of lipid blood biochemistry and blood coagulation indexes was analyzed by several treatment methods.Results Compared with the original blood samples, the serum biochemical indexes were significantly changed after adding fat emulsion, and the difference was statistically significant(P 〈 0.05). After the process of organic solvent ether, ALT, AST, TP,CK-MB and Cr had evident improvement. The hemagglutination indicators of the PT and APTT of plasma samples couldn’t be tested after adding the fat emulsion but could be tested after the process of N-hexane.Conclusion Taking the effective measures to improve the quality of hyperlipidemic specimens and eliminate the interference on the indexes of clinical biochemistry and hemagglutination so as to avoid sending the wrong test report and resulting in clinical physician’ misdiagnoses has the important clinical significance.
