摘要
目的观察个体化治疗老年退变性肩袖损伤患者的临床疗效。方法选取自2015年1月至2017年12月于沈阳市骨科医院诊断为"肩袖损伤"的老年患者90例为研究对象。所有患者拍摄术前MRI、术后MRI或DR,根据损伤程度分为轻度、中度、重度及巨大撕裂。对于轻度撕裂患者,仅予以髓质激发技术治疗;中度撕裂患者采用髓质激发+单排缝合治疗;重度及巨大撕裂患者采用髓质激发+双排缝合或缝线桥技术治疗。利用Constant-Murley评分评估患者术前及末次随访时肩关节疼痛、功能活动、活动度及肌力情况。结果末次随访时,患者疼痛及肌力减弱症状较术前明显好转,肩关节功能活动及活动范围得以改善,轻度、中度及重度损伤患者末次随访的Constant-Murley评分总分均较术前显著升高,差异均有统计学意义(P <0. 05)。结论肩关节镜个体化治疗老年肩袖损伤患者的技术可行,临床疗效可靠。
bjective To observe the effect of individualized treatment of elderly degenerative rotator cuff tears.Methods A retrospective study was performed on 90 cases of patients with rotator cuff tears who were admitted from January 2015 to December 2017.All patients were photographed preoperative MRI,postoperative MRI or DR.Patients were classified into moderate,severe and huge tears,according to the severity of the injury.For patients with mild laceration,single pulp stimulation technique was used.Patients with moderate tearing were treated with intramedullary stimulation+single row suture.Patients with severe and large lacerations were treated with intramedullary stimulation+double row suture or suture bridge.The patients′ shoulder joint pain,functional activity,activity level and muscle strength during preoperative and final follow-up were evaluated by Constant-Murley score.Results At the final follow-up,the patients with pain and reduced muscle strength were significantly better than those before surgery,and the functional activities and range of the shoulder joints were improved.The total score of the Constant-Murley scores increased significantly,with statistically significant differences( P 〈0.05).Conclusion The shoulder arthroscopy individualized treatment of senile rotator cuff injury patients has a feasible technology and reliable efficacy.
