急性胸痛患者呼叫急救医疗服务的分布特征和院前延迟现状 被引量:9

摘要 目的探讨北京地区急性胸痛患者呼叫急救医疗服务(emergencymedicalservices,EMS)的分布特征及院前延迟现状。方法连续人选2014-01-01-2015-12-31期间北京地区呼叫某EMS的急性胸痛患者,通过调取EMS调度中心的全智能数字化指挥平台数据精确获取患者的社会人口学特征、呼叫EMS的地点、呼叫EMS的时间和院前救治时间点。院前延迟细分为患者延迟和EMS转运延迟。结果北京地区呼叫某EMS的急性胸痛患者共28090例,占所有呼叫EMS患者的4.7%;随着年龄的增长,呼叫EMS的急性胸痛患者增多。城区发病的胸痛患者呼叫EMS更多(72.0%),冬季、工作日、晨起和傍晚是急性胸痛患者使用EMS的高峰。急性胸痛患者院前延迟的中位时间为180(113,720)min,其中患者延迟的中位时间为96(24,662)min,EMS转运延迟的中位时间为78(58,101)min,分别占院前延迟的53.1%和46.9%,而交通时间(74.6%)是EMS转运延迟的主要部分。结论急性胸痛患者呼叫EMS具有地区差异和时间节律,患者延迟和交通延迟是影响院前救治的主要环节,提示加强患者宣教和改善交通可能在一定程度上缩短院前延误。 Objective To explore the characteristics and pre -hospital delay in patients who dial for emergency medical services (EMS) due to chest pain. Methods Patients with acute chest pain dialing for EMS in Beijing were consecutively enrolled between January 1, 2014 and December 31, 2015. Data was collected through consulting the fully intelligent digital command platform of EMS dispatching center, including demographic characteristics, the disease spot, time of dialing for EMS and key time points of pre - hospital care. Pre - hospital delay was composed of patients delay and EMS transport delay. Results A total of 28090 patients with acute chest pain called for EMS in Beijing, accounted for 4.7 percent of all EMS calls, and the number of patients calls for EMS increased with age. Among the patients calling for EMS, 72% were located in the urban area, and most of patients made a call during winter, working day, morning and evening. The median pre - hospital delay was 180 (113, 720)rain, of which the median patient delay was 96 (24, 662) rain, and the median EMStransport delay was 78 (58, 101 ) min, respectively accounting for 53. 1% and 46.9% of the pre - hospital delay. While traffic time was the major part of EMS transport delay. Conclusion Patients with acute chest pain dial for EMS with regional differences and time rhythm, and patient delay and traffic delay are the main factors affecting pre - hospital care, suggesting that strengthening patient education and improving traffic conditions may reduce the pre -hospital delay to a certain extent.
