摘要
目的探讨中西医结合(前列地尔序贯疗法联合复方丹参滴丸)治疗老年糖尿病肾病的临床效果。方法随机选取本院于2016年6月～2017年11月收治的老年糖尿病肾病患者78例作为研究对象,根据其入院序号奇偶数按比例(1∶1)划分为对照组和观察组(每组39例),两组患者均接受常规治疗,对照组在此基础上行复方丹参滴丸治疗,观察组行前列地尔序贯疗法联合复方丹参滴丸治疗,观察对比两组患者尿微量白蛋白、尿总蛋白和肾动脉阻力指数情况。结果两组患者治疗前尿微量白蛋白、尿总蛋白比较差异无统计学意义;治疗3个月后,两组患者上述指标均明显降低且观察组低于对照组(P<0.05);两组患者治疗前主肾动脉、段动脉和叶间动脉阻力指数比较差异无统计学意义;治疗3个月后均呈现下降趋势,且观察组低于对照组(P<0.05)。结论针对老年糖尿病患者,给予前列地尔序贯疗法联合复方丹参滴丸治疗效果显著,可降低尿蛋白水平,促进肾脏动脉阻力指数恢复,值得进一步推广。
Objective To explore the clinical effect of combination of traditional Chinese medicine and Western medicine （alprostadil sequential therapy combined with Compound Danshen dripping pills） on senile diabetic nephropathy. Methods 78 elderly patients with diabetic nephropathy admitted to Liaoyang central hospital from June 2016 to November 2017 were randomly selected as subjects. According to the odd and even numbers of admission numbers, the patients were divided into control group and observation group （39 cases in each group）. Both groups received routine treatment. On the basis of above treatment, the control group was treated with Compound Danshen Dropping Pills and the observation group was treated with Alprostadil sequential therapy combined with Compound Danshen Dropping Pills. The microalbuminuria, total proteinuria and renal artery resistance index in two groups were observed and compared. Results There was no significant difference in urinary albumin and total urinary protein between the two groups before treatment. After 3 months of treatment, the above indexes of the two groups were significantly lower than those of the control group （P〈0.05）. There was no significant difference in resistance index of aorta and renal artery, segmental artery and interlobar artery between the two groups before treatment. After 3 months of treatment, they showed a downward trend, and the observation group was lower than that of the control group （P〈0.05）. Conclusion For elderly patients with diabetes mellitus, alprostadil sequential therapy combined with Com- pound Danshen Dropping Pills has a significant therapeutic effect, which can reduce urinary protein level and promote the recovery of renal artery resistance index, and is worthy of further promotion.
