摘要
目的 Disposable LMA喉罩用于全麻下乳腺区段手术中气道管理,3号喉罩和4号喉罩术后并发症的比较。方法选取2016年10月~2017年10月间在我院全麻下行乳腺区段切除术患者100例,体重50~70 kg,身高150~170 cm,随机分为A组(3号喉罩组)和B组(4号喉罩组)。记录两种型号喉罩的气道漏气压、置入时间、置入成功率、术中气道压、手术时间,并记录喉罩拔除后并发症(口腔黏膜损伤出血、术后咽痛及咽部不适)的发生情况。结果 A组置入时间明显低于B组,差异有统计学意义(P<0.05);两组置入成功率、气道压、口咽腔漏气压差异无统计学意义(P>0.05)。A组口腔黏膜损伤出血的发生率明显低于B组,差异有统计学意义(P<0.05)。术后咽痛和不适感的发生率A组明显低于B组,差异有统计学意义(P<0.05)。两组均未出现气道痉挛和误吸等并发症。结论乳腺区段手术女性患者选取3号喉罩更合适,可减少术后并发症的发生。
Objective To compare the postoperative complications of disposable LMA No.3 laryngeal mask and No. 4laryngeal mask in airway management in the mammary gland during general anesthesia. Methods 100 patients withmammary segmentectomy from the Department of General Anesthesia of Zhejiang Cancer Hospital October 2016-2017.10 were selected. The weight was 50-70 kg and the height was 150-170 cm. They were randomly divided intogroup A （No.3 mask group） and group B（No.4 mask group）. The airway leakage pressure, insertion time, insertion success rate, intraoperative airway pressure, and operative time of the two models of laryngeal mask was recorded. Thecomplications after the removal of the laryngeal mask（bleeding of oral mucosa, postoperative sore throat and pharyngealdiscomfort） were recorded. Results The insertion time of group A was significantly lower than that of group B, and thedifference was statistically significant（P〈0.05）. There was no significant difference in the success rate of insertion, airway pressure, oropharyngeal cavity leak pressure between the two groups（P<0.05）. The incidence of oral mucosal injuryand hemorrhage in group A was significantly lower than that in group B, and the difference was statistically significant（P〈0.05）. The incidence of postoperative sore throat and discomfort was significantly lower in group A than that ingroup B, and the difference was statistically significant（P〈0.05）. There were no complications such as airway spasm and aspiration occurred. Conclusion The No. 3 laryngeal mask used in female mammary segmentectomy patients is moreappropriate, which can reduce the occurrence of postoperative complications.
