摘要
目的分析早期胃癌(early gastric cancer,EGC)伴胃黄色瘤(gastric xanthelasma,GX)患者的一般临床特点,并探讨其临床意义。方法收集2009年1月至2017年6月在我院消化内镜中心初次行胃镜检查(排除胃、食管、十二指肠恶性肿瘤及术后放化疗者)且年龄≥40岁非癌患者共51 780例,其中GX患者528例。初次行胃镜检查的EGC患者共179例,把EGC患者分为伴GX组和不伴GX组,统计两组的年龄、性别、胃黏膜萎缩程度、幽门螺杆菌感染(H. ylori)情况,以及EGC伴GX在胃部的分布情况。结果 GX在EGC患者中所占的比率大于在非癌患者中所占的比率(P <0. 05)。EGC合并GX的发生与患者性别、H. pylori感染、胃黏膜萎缩程度有显著相关性(P <0. 05)。多因素Logistic回归分析显示,男性是EGC合并GX的独立危险因素(OR=2. 367,95%CI:1. 2286~1. 3636,P=0. 042)。结论 GX可能成为EGC发生的一个警告信号。
Objective To study the general clinical characteristics of the early gastric cancer（ EGC） combined with gastric xanthelasma（ GX）,and to explore its clinical significance. Methods A total of 51 780 patients who the first time to do a gastroscope） were collected from Jan. 2009 to Jun. 2017 in our digestive endoscopy center（ excluding the esophagus cancer,gastric cancer,duodenal tumor,radiotherapy and chemotherapy after the operation）,including 528 cases of GX. And in the patients,age was greater than or equal to forty years old. The clinical data of 179 patients with EGC were retrospectively analyzed. The EGC patients were divided into two groups： combined with GX group and noncombined with GX group,and the general clinical features of two groups were analyzed such as age,gender,the degree of gastric atrophy and Helicobacter pylori infection,and the distributions of EGC combined with GX in the stomach were analyzed. Results The proportion of GX in EGC was higher than that in non-cancerous patients（ P〈0. 05）. The occurrence of EGC complicated with GX was significantly related to the sex of the patients,H. pylori infection,and the degree of gastric mucosa atrophy（ P〈0. 05）. Multivariate Logistic regression analysis showed that men was the independent risk factor for EGC complicated with GX（ OR = 2. 367,95 % CI： 1. 2286-1. 3636,P = 0. 042）.Conclusion GX may be a warning sign for the presence of EGC.
