摘要
目的:观察灯盏生脉胶囊联合Epley耳石复位法治疗后半规管良性阵发性位置性眩晕的疗效和对生活质量的影响。方法:将132例诊断为后半规管良性阵发性位置性眩晕的患者随机分为对照组66例和观察组66例,对照组给予Epley耳石复位法治疗,观察组患者在对照组的基础上灯盏生脉胶囊治疗,两组疗程为4周。记录两组治疗后1周、1个月的临床疗效,观察两组治疗前后眩晕障碍量表(DHI),并随访治疗后6个月、12个月的复发情况。结果:观察组治疗1周后、1个月后的总有效率分别为81.8%,92.4%,对照组总有效率为69.7%,80.3%,观察组以上总有效率显著高于对照组(均P<0.05)。两组治疗1周后、1个月后的DHI量表的功能、情感、躯体单项及总评分较治疗前明显降低,观察组的单项及总分均低于对照组(均P<0.05)。观察组总复发率12.8%显著低于对照组27.3%(P<0.05)。结论:灯盏生脉胶囊联合Epley耳石复位法治疗后半规管良性阵发性位置性眩晕的疗效好,不仅能够改善患者生活质量,还能降低复发率。
Objective： To observe the effect of Dengzhan Shengmai capsule combined with epley otolith reduction on the treatment of benign paroxysmal positional vertigo after semicircular canal and its effect on quality of life. Methods： 132 patients with benign paroxysmal positional vertigo diagnosed as posterior semicircular canal were divided into control group （66 cases） and observation group （66 cases）. The control group was treated with epley otolith reduction method. The observation group patients were in the control group. Based on the treatment of Dengqi Shengmai capsule, the course of treatment is 4 weeks. The clinical efficacy of the two groups was recorded at 1 week and 1 month after treatment. The vertigo disorder scale （DHI） before and after treatment was observed, and the recurrence was followed up at 6 months and 12 months after treatment. Results： The total effective rate of the observation group was significantly higher than that of the control group after 1 week and 1 month （ P 〈0.05）. The function, emotion and body of the DHI scale after 1 week and 1 month after treatment. Single and total scores were significantly lower than before treatment. The single and total scores of the observation group were lower than those of the control group （ P 〈0.05）. The total recurrence rate of the observation group was significantly lower than that of the control group （ P 〈0.05）. Conclusion： Dengzhan Shengmai capsule combined with epley otolith reduction is effective in treating benign paroxysmal positional vertigo after semicircular canal treatment. It can not only improve the quality of life of patients, but also reduce the recurrence rate. It is worthy of clinical recommendation.
